AbstractThis paper extends the analyses performed in part one to the area of complex intrafamily comparisons. Ratings by patients are consistently elevated compared to ratings by parents on all scales of the Family Assessment Measure (FAM), and there are significant interaction effects, which, when examined, suggest that patient scores improve while parental scores are relatively unchanged over the course of treatment in a day hospital. The clinical implications of these findings are discussed, and we attempt to clarify the role of the family therapist in the treatment of bulimia nervosa. © 1995 by John Wiley&Sons, Inc

AbstractA relationship between dissociative experiences and the eating disorders has been reported. However the nature of that link is not clearly understood. This study investigates the links between dissociation (as measured by the Dissociative Experiences Scale ‐ DES II) and eating psychopathology in a clinical group of bulimic women and a nonclinical group of undergraduates. In the nonclinical group, specific dissociative styles were found to be linked with bulimic attitudes on the Eating Attitudes Test. In the eating‐disordered group, there was an association between the scales of the DES II and frequency of binging. This association was primarily due to the effects of the dissociative style of absorption (e.g., daydreaming). Suggestions are made for further research, and the clinical implications of the findings are discussed. © 1995 by John Wiley&Sons,

AbstractEighty‐three obese subjects with binge eating disorder (BED) were compared with 99 obese subjects not meeting criteria for BED on the Toronto Alexithymia Scale (TAS). Overall, the subjects in our sample were not significantly alexithymic, the mean global TAS score being 62.8 (SD = 10.2) which is comparable with the values found in nonpatient control samples. Furthermore, the mean TAS scores did not differ between obese subjects with and without BED. However, we found a slightly higher prevalence of alexithymia (TAS total score 74 and above) in BED subjects compared with non‐BED subjects (24.1% and 11.1%, respectively). A series of stepwise multiple regression analyses were run, exhibiting a significant relationship between the TAS and educational level and the Eating Disorder Inventory (EDI) subscales Interpersonal Distrust and Ineffectiveness. Age, body mass index, measures of depression, and eating pathology did not predict TAS scores. © 1995 by John Wiley&Sons,

AbstractSymptoms of anxiety can be prominent during treatment of bulimia nervosa Our experience is that bulimics who abuse laxatives have the most prominent symptoms of anxiety. We conducted ratings of anxiety in 23 bulimics who purge with laxatives and 17 who purge by vomiting. We found that the laxative‐abusing group had higher levels of state but not trait anxiety and that they were more likely to be treated with medication for anxiety during hospitalization. These data suggest an association between laxatives and anxiety in bulimia nervosa. © 1995 by John Wiley&Sons, I

AbstractAlthough it is well accepted that most patients with anorexia nervosa are perfectionistic, little work has been done in characterizing this behavior. Anorexics were assessed by two new multidimensional instruments that were designed to measure multiple aspects of perfectionism. Both scales confirm that underweight, malnourished patients with anorexia nervosa are perfectionistic. Importantly, elevated perfectionism scores persisted after weight restoration. Anorexics experienced their perfectionism as selfimposed, and not as a response to other's expectations. Perfectionism is a dimension of the rigid, obsessive behaviors that may contribute to resistance to treatment and relapse in anorexia nervosa. Thus, these scales may be of value in assessing response to treatment. © 1995 by John Wiley&Sons, Inc

AbstractBinge eating is a common problem among obese individuals, and a simple, accurate way to identify obese binge eaters is needed. This study measured the concordance of the Binge Eating Scale (BES) and the Eating Disorder Examination (EDE). Women seeking obesity treatment (N = 126) were identified as binge eaters or nonbinge eaters using the BES, and then interviewed by clinicians blind to BES score using the EDE, a semistructured interview considered to be the “gold standard” for eating disorder diagnosis. The BES accurately identified nonbinge eaters; 39 of 42 (92.9%) BES‐identified nonbinge eaters were confirmed by the EDE. However, the BES did not accurately identify binge eaters; only 43 of the 83 (51.8%) BES‐identified binge eaters were confirmed by the EDE. Reasons for the discrepancy between the BES and the EDE in the identification of binge eaters were explored, and modifications to the BES that might improve its accuracy were considered. © 1995 by John Wiley&S

AbstractMany studies have investigated the relationship between obligatory runners and eating disorder characteristics. Unfortunately, many studies have failed to consider the effect of menstrual status among females in these studies. The present study examined the psychological status and eating characteristics among amenorrheic runners, eumenorrheic runners, and eumenorrheic sedentary women. The results of the study indicated that there were no significant differences between groups on any of the psychological measures but there was a subgroup of amenorrheic runners who scored in the extreme range on the depression and eating disorder measures. There may be a subgroup of amenorrheic runners who have a significant psychiatric disorder although it is unclear whether it is a major depression or anorexia nervosa. Longitudinal studies are needed to fully understand the interaction between excessive exercise, eating disorders, and depression. © 1995 by John Wiley&Sons, Inc

AbstractThe prevalence of obstructive sleep apnea (OSA) in women who were being seen for obesity assessment, rather than for assessment of sleep disturbance, was assessed. A consecutive series of referrals to an eating disorders clinic (n = 40) were studied in the sleep laboratory. Their body mass index (BMI) ranged from 29.4 to 66.9 kg/m2. Overnight polysomnograms were carried out, with respiration and oxygen saturation being monitored. Four women (10%), 1 postmenopausal and 3 premenopausal, had significant OSA. They tended to have a higher percentage of body fat and higher BMIs than the nonapnea group. These findings indicate that the prevalence of OSA is higher in the general population of obese women than is generally thought. They highlight the need for professionals who work with obese women to consider the possibility that some of the complaints of fatigue and tiredness may be a result of OSA. © 1995 by John Wiley&Sons, Inc

AbstractBody shape dissatisfaction and dieting have previously been reported in 9‐year‐old Caucasian girls. As British adolescents of Asian origin appear at least as vulnerable to eating disorders, the present study investigated these issues in preadolescent British Asian girls. Fifty‐five Asian and 42 Caucasian 9‐year‐olds completed assessments of dietary restraint, body esteem, body satisfaction, and body figure preferences. The study revealed a high priority for thinness in both the Asian and Caucasian girls. Their desire for thinness was present even though the Asian girls had a significantly lower body weight than their Caucasian peers. Both groups of high restraint girls expressed body shape discontent and lower body esteem. In addition, an association was found in the Asian sample between reported dieting and the cultural orientation of their family. These results demonstrate the wide appeal of thinness, which in combination with intercultural and intrafamilial conflict, may strongly influence eating and contribute to the development of eating disorders. © 1995 by John Wiley
