AbstractIn the search for more effective methods of psychological treatment in anorexia nervosa, there are a number of controlled trials evaluating the efficacy of different forms of treatment. Previous studies have shown that family therapy is the superior treatment for patients with an early onset and short duration of illness. In order to assess the impact and the effective components of family therapy, we conducted a pilot trial in which consecutive referrals of anorexia nervosa patients were randomly assigned to one of two forms of family treatment: family therapy (conjoint family sessions) or family counselling (separate supportive sessions for the patient and counselling for the parents). Changes taking place within the patient and the family were evaluated at regular intervals, while within and between group comparisons were made. Although tentative, it was found that, in the short term, there were few differences in terms of symptomatic relief between the two treatment groups. © 1992 john Wiley&Sons, Inc

AbstractTwo series of women, one presenting with eating disorders and one presenting with other diagnoses, have been studied with respect to their recollection of childhood sexual experiences with adults. The series were studied using the same methods and same definitions by the same group of investigators. The results were compared in order to test the hypothesis that there might be a special association between such experiences and later clinical eating disorder, over and above any association which might exist with psychiatric disorder in general. The results do not support such a special association, although it remains likely that childhood sexual experiences with adults increase the chance of later eating disorder. Equity about this issue has a place in the assessment of such patients. © 1992 john Wiley&Sons, Inc

AbstractFour hundred fifty‐five females from a community‐bgased, randomly sampled population were interviewed by telephone to estimate the prevalence of binge eating and to describe demographic and topographical characteristics of binge eating behaviour. Binge eaters were classified according to the DSM‐111‐R criteria for bulimia, excluding purging behavior. The estimate for subjects meeting criteria was 1.8%, while the percentage of females who met all but frequency criteria was estimated at 3.8%. The majority of binge eaters in this study was found to engage in behaviors typically associated with binge eating episodes. On average, this group of binge eaters was older and heavier than has been reported previously, which supported observed associations between binge eating and overweight in clinical populations [Telch, Agras,&Rossiter (1988). AbstractA case series of four patients with disorders of gonadal differentiation and maturation who subsequently developed bulimia nervosa is described. To the authors' knowledge these are the first reports of bulimia nervosa in conjunction with preexisting gonadal pathologies. AbstractFindings indicate that bulimics with Borderline Personality Disorder (BPD) may represent a distinct subgroup, with extreme psychopathology and unique treatment needs. However, since mood disturbances can produce “borderline” manifestations, findings from available borderline/nonborderline comparisons in bulimic samples could confound features associated with personality disorder (PD) with those associated with mood disturbance. We compared psychiatric and personality features in 91 bulimics, divided (using DSM‐III‐R criteria) into those with BPD, Other PDs, or No PD. In direct comparisons, cases meeting “border‐line” criteria seemed broadly more disturbed than did cases with other Axis‐II comorbidity patterns. However, when variance attributable to depression was accounted for, greater psychopathology in “borderlines” became restricted to focal dimensions, predictably characterizing a “hostile‐impulsive” phenotype. While “borderlines” displayed higher prevalences of certain Axis‐l syndromes than did “nonborderlines”, results did not indicate distinct patterns of comorbidity, on Axis I, across groups. AbstractThis study examines aspects of the relationship between religious belief and anorexia ner‐vosa. It uses data from postal questionnaires sent to members of a U.K. national self‐help organization for people with “eating disorders” which elicited a profile of symptoms and other clinical data and information about personal and family religious beliefs. The data suggest that the majority of respondents were or had been afflicted with anorexia nervosa. Subjects with a religion, particularly those with strong beliefs, and particularly those who were Anglican, reported particularly lowest ever adult Body Mass Indices (BMIs). Part of the explanation for these findings would seem to be an increase in the importance of subjects' religious beliefs during their anorectic illnesses. Conversely, bulimic symptomatology seemed to be associated with a weakening of subjects' beliefs. Religious conversion seemed to serve as a protective function against severe weight loss. © 1992 John Wiley&S

AbstractThe Eating Disorder Inventory (EDI) was used in a transcultural study of East and West Berlin clinical and nonclinical samples at a time when the two parts of the city were still strictly separated. East Berlin patients with anorexia nervosa scored significantly lower than their Western counterparts with regard to the majority of EDI scales. Furthermore, there was a lack of discriminant validity for the ‐East Berlin clinical and nonclinical samples. It was speculated that this deficit might reflect a lack of transcultural validity in the psychological constructs upon which the questionnaire is based. However, the study revealed some evidence of meaningful correlations with clinical parameters. Finally, evidence was presented supporting internal consistency of the subscales and convergent validity of the EDI with the Eating Attitudes Jest (EAT). © 1992 John Wiley&Sons, I

AbstractPast studies on the behavioral effects of dieting in overweight individuals have produced conflicting results. Overweight restrained eaters and self‐initiated dieters have eaten little whether preloaded or not, whereas overweight individuals who have lost weight in weight reduction programs have eaten a lot whether preloaded or not. The present study examined the effects of requiring overweight self‐initiated dieters to stay on or “go off” their diets for 2 days. A control group of normal weight self‐initiated dieters was subjected to the same dieting manipulation. All subjects then participated in an ostensible ice cream taste test. Overweight subjects who stayed on their diets ate almost twice as much as overweight subjects who went off their diets; the intake of normal weight subjects was unaffected by the dieting manipulation. A cognitive explanation was developed to account for the findings of the present study along with those of past studies on self‐initiated and weight‐loss dieting. © 1992 John W

AbstractIt has been noted that women with insulin‐dependent diabetes mellitus (IDDM) may be at increased risk to develop disordered eating because their medical regimen requires an intense focus on diet. Thus the goal of the present study was to determine the prevalence of eating disorders symptomatology in a registry‐based sample of women with IDDM and to assess the relationship between eating disorders symptomatology and blood sugar control. One hundred eighty‐eight women with IDDM completed the Bulimia Test (BULIT) and the Eating Disorders Inventory (EDI). BULIT and EDI scores of diabetic subjects were comparable to those of no diabetic subjects in standardization samples. However, eating disorders symptomatology among IDDM women was significantly associated with poorer control of diabetes. This study suggests that diabetic women are not more likely than nondiabetic women to report eating disorders symptomatology, but converging evidence indicates that these symptoms are associated with poorer control of diabetes. © 1992 John Wiley&Son

AbstractElevations of serum amylase have been reported in 25–60% of bulimics. This increase in amylase appears to derive from the nonpancreatic fraction of serum amylase and thus underscores its likely association with previous clinical descriptions of parotid gland enlargement among bulimics. The value of measurement of serum amylase as an index of clinical symptomatology in bulimia is currently limited because of substantial overlap of amylase levels in patients and normals, and because of variability in correlations between amylase levels and symptom severity. This article reviews the physiology of amylase secretion from the salivary glands and, in light of previous studies of physiologic alterations in bulimia, assesses factors that may contribute to variance in amylase levels in bulimics. Controlling for these variables in future studies may yield more useful clinical information. © 7992 lohn Wiley&Sons, I
