SUMMARYCellular DNA in skin biopsies from eighteen patients with positive patch tests and eleven patients with benign lymphocytic skin infiltates (cutaneous lymphocytoma, lymphoplasia or Jessner's lymphotic infiltrate) was estimated by flow cytometry of propidium iodide‐stained dermal cells.Specimens from three patients with patch tests and one with cutaneous lymphocytoma had DNA histograms similar to those of normal blood mononuclear cells. The remaining patients demonstrated DNA histogram abnormalities in the form of hyperdiploid peaks in forty‐two of forty‐five specimens. The estimated proportion of cells in the S‐phase was similar in patch tests (median:9.2%) and in benign lymphocytic skin infiltrates (median: 9.6%). The median number of cells in the G2/M‐phase was 3.1% in both groups of disorders.These data indicate DNA content heterogeneity of the dermal cells in benign lymphocytic skin infiltrates. This may be attributed to DNA replication and/or chromatin dispersion during cellular activation and proliferation. SUMMARYClinically uninvolved psoriatic epidermis shows increased DNA synthesisin vivo. We have studied the DNA synthesis of cultured keratinocytes from uninvolved psoriatic skin. Trypsinized epidermal cells were plated on plastic dishes pre‐coated with bovine collagen type I. In initial studies, normal human serum was found to be superior to fetal bovine in supporting the growth of human epidermal keratinocytes. Furthermore, keratinocyte cultures established in the presence of normal human serum produced large keratin proteins (68,000 daltons) indicating that the terminal steps in cell differentiation can occurin vitro. In subsequent experiments keratinocyte cultures were grown in medium supplemented with 10% normal human serum. Confluent cultures of keratinocytes from uninvolved psoriatic epidermis had an increased DNA synthesis determined both as the incorporation of [3H]thymidine and as the autoradiographic labelling index. The DNA synthesis of both normal and psoriatic keratinocyte cultures increased in response to incubation in medium with 10% psoriatic serum. The ability of keratinocytes from uninvolved psoriatic epidermis to maintain an increased DNA synthesis suggests the presence of an inherent defect within the population of epidermal keratinocytes in psoriasis. SUMMARYExtensive morphological and biochemical studies were carried out on two sibling cases of ichthyosis linearis circumflexa. The condition was found to be similar to psoriasis in the following ways: (1) effectiveness of PUVA therapy, (2) psoriatic changes on light and electron microscopy, (3) high urinary polyamine levels, (4) elevation of several enzyme activities in the scales, and (5) a remarkable change in keratin molecules. These results may reflect an increased epidermal proliferation with a reduced epidermal transit time, as occurs in psoriasis.

SUMMARYPlatelet activating factor (Paf‐acether, AGEPC) is a family of ether‐linked phospholipids known to be released from a range of inflammatory cell types.In vitroand in experimental animals, it seems to be a mediator of inflammation, and intradermal injection of Paf‐acether in man elicits a biphasic inflammatory response, reminiscent of the dual response to allergen in sensitized individuals. In the present study, cutaneous histology was assessed in sequential skin biopsies from six normal volunteers after intradermal injection of 200 or 800 pmol Paf‐acether. Paf‐acether (200 pmol) induced intravascular accumulation of neutrophils, accompanied by a p erivascular mixed cellular infiltrate which was composed predominantly of neutrophils at 4 and 12 hours, and lymphocytes and histiocytes at 24 hours. Control injections of lyso‐Paf and normal saline induced no noteworthy histological changes. Paf‐acether (800 pmol) resulted in vessel destruction, gross endothelial swelling and a perivascular infiltrate of mononuclear cells and neutrophils, accompanied by occasional evidence of leucocytoclasis. SUMMARYIntestinal permeability was investigated in adult patients with atopic eczema byin vivoandin vitrotechniques. Patients with symptoms of ‘immediate’ food allergy were specifically excluded. A51Cr‐labelled ethylenediaminetetraacetate absorption test was carried out in eighteen patients. Their mean (± s.d.) 24‐hour urine excretion following oral administration of the test substance (2·1 ± 0·9%) did not differ significantly from that of thirty‐four normal controls (1·9±0·5%). Small bowel permeability was estimated directly in jejunal mucosal samples in ten patients with three permeability probes of differing molecular weight. Mucosal permeability did not differ significantly from that of fifteen control patients for any of the test substances. Two patients had abnormal results by both tests and in one this was due to coeliac disease. These results suggest that altered intestinal permeability is not important in the pathogenesis of eczema. SUMMARYLocalized persistent pigmented macules and plaques due to a chronic form of pigmented purpuric dermatitis are described in forty‐two patients. The designation ‘lichen aureus’ for this eruption can be justified because of the striking yellowish or bronze‐like colour assumed by many of the lesions. SUMMARYThirty‐five patients with lupus pernio were observed in a series of 818 patients with clinical and histological evidence of sarcoidosis. This analysis provides the natural history of lupus pernio and its associated clinico‐radiographic features. There was intrathoracic involvement in 74% of patients, upper respiratory tract disease in 54%, reticulo‐endothelial involvement in 54%, bone cysts in 43%, and ocular lesions in 37%. It predominated in West Indies‐born women. Histological confirmation was easily obtained by nasal mucosal biopsy and this could be repeated serially to evaluate the response to various treatments. Nasal bone radiographs showed destruction of the nasal bones, porosis and alteration in bone texture.
