Summaryα1‐Antitrypsin phenotypes and trypsin‐inhibitory capacities were measured in fifty‐one patients with psoriasis. An increased number of variant phenotypes (MS, MZ, and SS) were found only in those patients with severe psoriasis (20% or more skin involvement) and not in those with lesser involvement. SUMMARYThe kinetic properties of phospholipase A2isolated from single large specimens of normal human epidermis and‘uninvolved’(lesion‐free) psoriatic epidermis were determined. The enzymes from the two sources behaved identically with respect to changes in protein concentration, Ca2+concentration and pH, but the enzymes responded differently to changes in substrate concentration. Furthermore, the specific activity of the enzyme derived from lesion‐free psoriatic epidermis was higher than that from normal epidermis under all conditions used. Increased specific activity of the enzyme in the lesion‐free epidermis was also found when biopsy specimens taken from thirty‐five patients with psoriasis vulgaris at varying severity were compared with biopsies of normal epidermis from thirty‐one control volunteers (P<0.001). Mixing experiments, in which homogenates of lesion‐free psoriatic epidermis and control epidermis were combined, suggested that the relatively low activity of the enzyme in normal epidermis was due to the presence of an inhibitor. As the activity of the enzyme was not elevated in the lesion‐free epidermis from twelve cases of eczema, which is also an inflammatory condition of the epidermis and superficial dermis, it is suggested that the raised phospholipase A2activity demonstrated in the lesion‐free epidermis of psoriasis may play an important role in the pathogen

SUMMARYTissue from the acute, non‐infected eruption of sixteen atopic eczema subjects was subjected to an indirect immunoperoxidase technique using monoclonal antibodies recognizing T‐lympho‐cyte subsets, Langerhans cells and natural killer cells. Over half the cells infiltrating the dermis were T lymphocytes, including a large majority of helper T cells and relatively few suppressor T cells. Langerhans cells were present in significant proportions in the dermis and probably reflected increased antigen presentation within the affected skin. SUMMARYIndirect immunofluorescence revealed the presence of pemphigus‐like antigenic activity on SCaBER cell monolayers after incubation with sera from patients with pemphigus vulgaris. In contrast, no fluorescence was observed after incubation of the cells with sera from patients with bullous pemphigoid, systemic lupus erythematosus or healthy individuals.In order to identify this pemphigus‐like antigen, the SCaBER cell membrane proteins were solubilized in non‐ionic detergent, separated according to their molecular weight by sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS‐PAGE) and electrophoretically transferred to nitrocellulose sheets. After incubation with pemphigus serum followed by125I‐radiolabelled goat anti‐human IgG, autoradiography revealed pemphigus antibody binding activity in the 105 Kd region in electrophoretograms of the unreduced SCaBER cell membrane extracts. SummaryThe duct of the human atrichial sweat gland, after thermal stimulation, exhibited increased cytoplasmic vesiculation, particularly of the luminal cells, a widening of the intercellular spaces, and the presence of particulate matter in the lumen. The luminal cells of the coiled zone also displayed varying degrees of apical disruption, most probably due to filtration of cell fluid through the terminal web of microfilaments. This zone seems to have a secretory as well as an absorptive role. Specialized junctions (intercellular bridges, gap junctions and tight junctions) were found amongst the basal and luminal cells of the wall of the ascending duct.

SummaryThe expression of cytokeratins in Paget cells in mammary and extramammary Paget's disease was studied using different keratin antibodies and immunofluorescence microscopy. Antibodies to epidermal keratin did not react with the Paget cells but stained the surrounding epidermis. Two monoclonal cytokeratin antibodies (PKKi and RGE 53), which reacted typically with simple glandular epithelia in normal tissues, brightly stained the Paget cells and left the surrounding epidermal cells unstained. The results indicate that Paget cells are derived from mammary or sweat duct epithelium rather than from epidermal cells.

SUMMARYThe histopathology of striae distensae is disputed; different authorities give contradictory accounts of the microscopic changes, especially in elastic fibres.We re‐evaluated the problem by taking eight elliptical biopsies across striae. Six were examined by light microscopy with appropriate stains for elastin and collagen. Two were prepared for scanning electronmicroscopy (s.e.m.), using a procedure which removes collagen, enabling the elastic network to be seen in its native form.By light microscopy, striae were sharply demarcated from normal skin, consisting mainly of fine, straight bundles of collagen arranged parallel to the surface. Fine elastic fibres were disposed similarly without fragmentation, fraying or curling.By s.e.m., the elastic network was found to be extraordinarily dense and well developed with many fine, curled fibres in random array. It was evident that the routine stains for elastin greatly underestimated the abundance of elastic fibres, probably because immature fibres contain insufficient protein matrix. The horizontal packing of collagen bundles was confirmed by s.e.m.These findings support the view that striae distensae are scars. SUMMARYWe have compared ultrasound and Harpenden calipers for the measurement of thickness of non‐inflamed and anthralin‐inflamed skin. In non‐inflamed skin ultrasound gave consistently and significantly lower values (0.87 mm ±2.3% s.e.) than calipers (1.1 mm±2.7% s.e.) and the ultrasound/caliper ratio was 0.89±1.3%. In anthralin‐inflamed skin the ultrasound/caliper ratio was not significantly different from unity (1.02±1.3%), but ultrasound was unable to measure inflamed skin thickness in 18% of cases because the echo could not detect the dermis‐subcutaneous fat interface. The proportion of unreadable ultrasound results increased linearly with increase in skin thickness and the variance of ultrasound readings increased as inflammatory skin thickness increased; by contrast caliper variance remained constant. Harpenden calipers cannot be used in all subjects or in skin sites in which skin folds cannot be raised, but if the patient and site to be studied can be selected, Harpenden calipers have a greater precision, reproducibility and reliability than ultrasound for measurement of the thickness of
