SUMMARYThe effect of various retinoids (retinol, all‐trans‐retinoic acid, etretinate, 13‐cis‐retinoic acid and an arotinoid, Ro‐13‐6298) on cultured human epidermal cells has been investigated. Cell growth was inhibited in a dose‐dependent fashion by retinol, all‐trans‐retinoic acid and 13‐cis‐retinoic acid, but to a lesser extent by etretinate. The arotinoid showed an enhancing effect on cell growth. The area of cells on the culture dish was reduced by all the retinoids in a dose‐dependent fashion, but the arotinoid was especially active in this respect even at very low concentrations. The measurement of cell volume by cell size frequency analysis, using a Coulter counter channelyzer, showed no differences compared with control cultures, and so the reduction of cell area was considered to be due to reduced ‘spreading’ of cells on the substrate. These results suggest that the retinoids interfere with cell‐to‐cell and cell‐to‐substratum attachment. SUMMARYWe compared the maturation pathway of normal and psoriatic epidermis using three different markers: (I) Involucrin, which is normally detected in the stratum granulosum in normal skin, was detected in all but the basal layer of involved psoriatic skin; (2) an antigen, recognized by the murine monoclonal antibody Ψ3, was present in all but the basal layer of involved psoriatic skin but was absent from uninvolved and normal skin; (3) fibronectin, which normally localizes in the dermis and the epidermal‐dermal junction, was also detected intra‐ and extracellularly in the psoriatic epidermis. SUMMARYCirculating polymorphonuclear neutrophils (PMNs) from 50 patients with psoriasis vulgaris and from 38 control individuals (28 healthy blood donors and 10 patients with systemic sclerosis) were tested for their ability to adhere to human endothelial cell monolayer and a plastic surface. We also studied the effect of sera from psoriatic patients on the adherence and chemotactic activity of normal human neutrophils. The neutrophils from patients with psoriasis were much more adherent to the endothelium and the plastic surface, and this correlated positively with the activity of the disease, but inversely with the extent of the skin lesion. The sera from patients with active psoriasis were found to enhance the adherence and chemotaxis of normal human PMNs. Treatment of the sera by heat (56°C for 30 min) did not affect their enhancing effect on the adherence but significantly decreased their effect on the chemotactic activity of normal PMNs

SUMMARYA radioassay method allowing measurements of low concentrations of circulating hyaluronate was used in a study of serum hyaluronate concentrations in 44 patients with psoriasis. Twenty‐three of them had only skin lesions and 21 had both skin lesions and arthropathy. In both of these groups significantly elevated serum levels of hyaluronate were found. The highest values were observed in those with widespred and active skin disease and/or active arthritis. Serum hyaluronate positively correlated with the plasma concentrations of α1‐antitrypsin and haptoglobin and also with ESR, which may indicate a relationship between acute inflammation and increased production of hyaluronate. Seven patients with widespread atopic dermatitis included for comparison had normal hyaluronate values. In blister fluid from lesional skin in two patients with acute psoriasis, very high concentrations of hyaluronate were found, in comparison with the concentrations in blister fluid from non‐involved skin. SUMMARYPlasma phospholipid essential fatty acids were investigated in 40 patients with leprosy and 40 controls. A significant reduction in linoleic acid was found in the leprosy patients, with an increase in its metabolite dihomo‐γ‐linolenic acid. SUMMARYThe prevalence of keratosis pilaris and accentuated palmoplantar marking was evaluated in 61 patients with atopic dermatitis, 35 patients with dominant ichthyosis vulgaris and 247 other dermatological cases taken as controls. Our data showed that (1) these features are of no diagnostic significance for atopic dermatitis and (2) they are significantly more frequent in patients with ichthyosis vulgaris without associated eczema than in those with atopic dermatitis. Consequently, they should be considered as part of the phenotype of ichthyosis vulgaris rather than attributed to a concomitant atopic dermatitis as suggested by some. These findings should be taken into account when evaluating atopic dermatitis or ichthyosis. To assess the frequency of scaling under winter weather conditions, 155 control subjects were also examined for evidence of visible desquamation and 25.8% showed slight but definite scaling.

SUMMARYAntibodies reactive with various layers of normal human epidermis can be detected in a high percentage of sera from normal adults. Stimulation to produce these antibodies occurs at a very early age, with some suggestion thatin uterostimulation may occur in a small number of cases.

SUMMARYThe merits of oral ketoconazole and griseofulvin in dermatophytosis have been compared in a double blind study on 74 patients with 152 infected sites. The initial daily doses were 200 mg and 500 mg respectively, but these were doubled after 3 months if there was an inadequate clinical response. Treatment was continued either until clinical and mycological remission was achieved or a year of therapy had been given. Seventy‐five percent (total 80) and 74% (total 72) of all infected sites treated with ketoconazole and griseofulvin respectively were cleared of infection. However, in toe nail infections the respective cure rates were only 21% and 17%. Ketoconazole appeared to act more rapidly in curing tinea corporis or tinea cruris due toTrichophyton rubrum, whereas griseofulvin was superior inT. interdigitaleinfections. No serious side‐effects were encountered in either treatment group. In view of the slight risk of drug‐induced hepatitis, ketoconazole is best reserved as a second‐line drug for toe nail infections unless there are specific indications, such as griseofulvin intolerance. In these cases liver function tests should be monitored regularly throughout

SUMMARYForty patients with the cutaneous T‐cell lymphoma (mycosis fungoides) were treated with total skin electron beam irradiation. The total follow‐up period was up to 116 months (median 57.5 months). The patients were irradiated with a total dose of 35 Gy over 10 weeks, using the six‐field technique. Ten of these patients had lymph node involvement and were subsequently treated with chemotherapy. After the initial electron irradiation complete remission of the skin lesions was obtained in 87.5% of the patients. Relapse of skin lesions occured in 52% of the patients after 2–72 months (median 4 months). Second line therapy consisted primarily of topical nitrogen mustard.The overall survival rate at 5 years was 70%. Despite the side‐effects this treatment was tolerated well by all
