SUMMARYA review of sixty‐four skin biopsies from sixteen patients with lymphomatoid papulosis revealed two characteristic histological types (type A and type B), which also had a different clinical behaviour. In lymphomatoid papulosis type A lesions, four histological patterns corresponding with the age of the lesion could be distinguished. Such a relationship was not found in type B lesions. The finding of transitional forms in some biopsy specimens, showing histological features of both type A and type B, and the presence of both types in different but concurrent lesions, suggests that these two types are not different entities but rather represent the ends of a spectrum.At least two different populations of atypical cells can be distinguished in lymphomatoid papulosis. Apart from the atypical cerebriform mononuclear cells, which are T‐lymphocytic in origin and predominant in type B lesions, large atypical cells with vesicular nuclei, prominent nucleoti and abundant cytoplasm are found, particularly in type A lesions. SUMMARYClinicopathological findings, methods of treatment and survival in ninety‐two patients with cutaneous T‐cell lymphoma (CTCL) (eighty‐five mycosis fungoides and seven Sezary syndrome), seen in the Leiden University Hospital between 1974 and 1980, are reported. All patients were staged and treatment was given accordingly. Patients without signs of extracutaneous disease (74%) were treated with either total‐skin electron‐beam irradiation (E beam), topical mechlorethamine (HN2) or PUVA. Topical HN2and E beam had an equal effect on survival. Electron‐beam induced a higher number of initial complete remissions than HN2, but fewer patients relapsed while on maintenance treatment with HN2.In the patients with lymph node and/or visceral involvement, E beam followed by systemic chemotherapy—cyclophosphamide, vincristin (Oncovin) and prednisone (COP)—gave better results than topical therapy or COP alone. SUMMARYGranulocyte and monocyte functions (phagocytosis, intracellular killing, chemokinesis and chemotaxis) and the opsonic and chemotactic activity of the serum of twenty‐two patients with cutaneous T‐cell lymphoma were assessed. Granulocyte functions were within the normal control range in most cases. The monocyte functions showed more variation in the test results; in six out of twenty‐two patients intracellular killing ofStaphylococcus aureuswas depressed and in four out of ten patients a disturbance in monocyte chemotaxis was found. SUMMARYIn this report we describe significant alterations in the appearance of peripheral blood granulocytes (PMNs) from psoriasis patients when compared with PMNs from normal control subjects. Psoriasis PMNs are polar in shape and exhibit ruffled cell membranes.

SUMMARYIn patients undergoing photochemotherapy (PUVA) a selective accumulation of lipid droplets was observed within the cytoplasm of melanocytes. Keratinocytes did not develop lipid droplets. Within 2 months after clearing of psoriasis the droplets gradually vanished and did not reappear even during maintenance PUVA therapy.Increased intracellular lipid deposits are usually taken to herald cell degeneration. Since the maximum lipid accumulation coincided with the end of the clearing phase when melanin production was at its height, the intramelanocytic lipid may have been a morphological sign of over‐stimulation of melanocytes, which could eventually result in melanocyte destruction. Since excess melanin may be cytotoxic for melanocytes this may explain the irreversible hypopigmentation which develops in some patients treated with PUV

SUMMARYTitres of anti‐stratum corneum (SC) antibodies, determined by an indirect immunofluores‐cence (IF) technique, were significantly higher on average in pustulosis palmaris et plantaris (PPP) and pompholyx than in normal controls. By a complement IF technique, the antibodies fixed complement (C3) in high title in PPP, but to a much lesser degree in pompholyx and normal controls. Direct IF microscopy studies showed the deposition of IgG and C3in the SC in some of the biopsy specimens of the pustular lesions. SUMMARYWe studied biopsies from twenty‐nine patients with pityriasis rosea. All specimens showed features of subacute or chronic dermatitis as previously reported, but sixteen cases (55%) had dyskeratotic cells within the epidermis. SUMMARYThirty two adult patients who presented with a sharply marginated, hyperkeratotic dermatitis of the palms (HDP), but with no skin lesions elsewhere, have been re‐examined approximately 10 years later. Topical treatments had had limited effect. The morphological appearance and localization had remained unchanged, except in two individuals who were free from symptoms. One had developed psoriasis. SUMMARYTwenty‐five patients with rosacea were compared with twenty‐five control subjects for previous medical history and tests of immune function. Rosacea patients were found to have a higher incidence of disorders of the auto‐immune type and were significantly more difficult to sensitize to DNCB than the controls. In addition, twelve of the rosacea patients and eleven other rosacea patients had biopsies which were examined by the direct immunonuorescence technique. In only five was the test negative. In the remainder deposits of IgM and/or IgG and/or complement were found at the dermo‐epidermal junction and/or in the dermal collagen. Serum from the rosacea patients was also examined by the indirect technique and in six cases a circulating antinuclear antibody of IgM type was found. SUMMARYThe effects of potassium iodide, colchicine and dapsone on thein vitrogeneration of polymorphonudear leukocyte (PMN)‐derived oxygen intermediates were investigated. These three drugs have beneficial effects on those conditions in which PMNs play an important pathogenetic role. Three oxygen intermediates, superoxide anion (O2−), hydrogen peroxide (H2O2), hydroxyl radical (OH·) and chemiluminescence were included in assay studies. Dose response studies were performed with therapeutic doses of the drugs (10 μM–1 mM). We found that both potassium iodide and dapsone significantly suppressed the generation of oxygen intermediates, except for O2−. Colchicine decreased OH · production.Our results show that these agents to some extent exert their anti‐inflammatory effects by interfering with the PMN‐dependent production of oxygen intermediates, thus conferring protection from auto‐oxidative tissue injury.
