SUMMARYMast cell population density was determined in normal skin from two regions of the arm of several healthy men and compared with blood vessel density and histamine concentration in the same sites. Mast cell and hlood vessel counts were made in 1–1.5 μm thick plastic sections, by light microscopy and tissue‐histamine concentrations were determined by automated fluorimetric analysis.Statistically significant correlations were found between mast cell counts, blood vessel counts and histamine content in skin from the upper arm but no similar correlations were obtained in the forearm. Anatomical differences between the two sites may have been the cause of this discrepancy. Wide variations in mast cell counts and blood vessel density were found in different sections from the same biopsy samples which confirms the notion that dermal mast cells are imevenly distributed. Analysis of variance of the mast cell counts showed that the variance between sections from different blocks from the same biopsy samples was greater than the variance between adjacent biopsies. SUMMARYA new method was developed to define the distribution of mast cells in normal human skin. The population density was determined in a number of zones of different depth in the dermis. Two regions of the same limb were studied systematically. Both were shown to have mast cell populations of similar size and distribution, with a maximum density immediately below the dermo‐epidermal junction, gradually falling to a minimum density in the deeper layers of the dermis. Small secondary peaks were less clearly defined at the base of the dermis.These findings should provide baseline data for comparisons with mast cell population size and distribution in different areas of normal skin, or in similar areas in clinical disorders or experimental condition

SUMMARYStudies of the sweating mechanism in juvenile plantar dermatosis have demonstrated features of miliaria, including inhibition of sweating and occlusion of the sweat coil in the horny layer of the epidermis. These are considered to be a sequel to changes in the composition of children's shoes and hose. Possible remedies are suggested.

SUMMARYA new post‐surgical form of recurrent herpes simplex is described. It appeared as an erythematous bulla at sites of recurrent herpes from which the prior epidermal lesion had been transversely excised. Histologically, it was characterized by a lesion‐free epidermis, a large subepidermal bulla, and a marked inflammatory response in the uppermost dermis, complete with diagnostic multinucleate giant cells. It is postulated that epidermal surgery leaves the area immune to future direct herpetic involvement of the epidermis because it denervates the epidermis. SUMMARYForty‐six patients with xanthomatosis and elevated very low density hpoproteins (VLDL) levels (in different types of hyperlipoproteinaemia) were classified on the basis of the WHO criteria and the cholesterol, triglyccride ratio in VLDL. A large majority (31/46) of the patients referred to the Department of Dermatology could be classified as hyperlipoproteinaemia type III, only 8/46 as typeIIBand 7/46 as type IV/V.This distinction seems to be relevant as the xanthomatous lesions differed distinctly between these three types of hyperlipoproteinaemia. Xanthochromia striata palmaris was present in 29/31 cases of hyperlipoproteinaemia type III and was not found in type IV/V patients, who had distinctive papulo‐ eruptive xanthomas. During a follow‐up in 35/46 patients all xanthomas disappeared within 2 years except the xanthelasma palpebrarum and tendinous xanthomas. All type IV/V patients (7/7) but only one typeIIIpatient (1/31) had abnormal glucose tolerance. Only 2/18 type III patients less than 45 years showed claudication and none of the young type III patients had angina pectoris. In contrast, all four typeIIBpatients less than 45 years had clinical signs of atherosclerosis. However, angina pectoris and/or claudication were present in 5/13 type III patients over 45 years old. The mean serum cholesterol level was equally elevated in both groups but the cholesterol was mainly present in VLDL in type III and in low density lipoproteins (LDL) in typeIIB. In 9/31 type III patients theLDLlevel was also elevated but was easily normalized by a diet low in carbohydrate, whereas the elevated LDL level in type IIB was therapy‐resistant. The recognition of xanthomatous lesions, specifically xanthochromia striata palmaris, as an early sign of type III hyperlipoproteinaemia, can lead to the early diagnosis and successful treatment of these patients, and thus possibly prevent the development of premature atherosc

SUMMARYDuring the past 2 years we have studied eighty‐nine biopsy specimens from nineteen patients with Cowden's syndrome. Among fifty‐three facial lesions, twenty‐nine were trichilemmomas. Twenty‐ three of the others were consistent with trichilemmoma, showed a non‐specific benign verrucous acanthoma, or were not diagnostic; one was a bluenaevus. All fourteen oral mucosal biopsy specimens were benign fibromas. Nineteen of twenty‐two biopsy specimens from the hands and feet showed the pattern of benign keratosis. Multiple trichilemmomas were found in all patients with Cowden's syndrome, but at times several biopsy specimens were required before a diagnostic picture was un‐ covered. All patients with multiple facial trichilemmomas were found to have Cowden's syndrome. SUMMARYThere is evidence that anti‐androgens may help in the treatment of acne vulgaris. We therefore performed a double‐blind trial to assess the effect of topically applied progesterone on sebum excretion rate. Thirty‐eight patients took part in the trial which was for 3 months. SUMMARYThe dehydroepiandrosterone hydroxylatmg enzyme system in the hair follicle has been characterized. It is found in the particulate fraction of the follicle, and is inhibited by carbon monoxide and the cytochrome P450 inhibitor metyraponc, and appears to be dependent upon NADPH. In these major characteristics the hair follicle enzyme is very similar to the liver monooxygcnase system.
