ABSTRACT:Rheumatoid arthritis is described by Swanson (1995) as ‘the greatest crippler from the standpoint of severity and prolonged disability’ (p. 1307). One key factor in keeping persons with rheumatoid arthritis independent in their activities of daily living is maintaining functional use of their hands. In rural areas, where a move to assisted accommodation may require a person to leave the locality, this splint may help older citizens to remain functional and at home. The deformity and instability caused in rheumatoid arthritic hands by radial deviation of the wrist and subsequent ulnar deviation of the metacarpophalangeal (MCP) joints can greatly affect functional use of the hands. In this article, design and fabrication instructions for a lycra working splint which is suitable for the rheumatoid arthritic hand with MCP ulnar deviation are presented. ABSTRACT:The child with a fever is a common but sometimes difficult problem. The recognition of a serious bacterial infection is usually possible with careful clinical assessment. Occult bacteraemia may be missed, and apian of examination, investigation and management is outlined. The use of antibiotics and antipyretic treatment is discussed.

ABSTRACT:Among New Zealand occupational injuries, farm‐related injuries are second only to forestry in importance. A three‐year Farm Safety Study, funded by the Accident Rehabilitation and Compensation Insurance Corporation, researched the epidemiology of farm injury in New Zealand, the safety behaviour and attitudes of people living on farms and the international literature on farm safety strategies. The Farm Safety Study made funds and regional injury information available to a local farm women's network in Southland to initiate work on farm safety through a series of workshops. Evaluation indicated effective coverage of farm types and localities. Outcomes included increased safety awareness, suggestions for a range of individual, community and institutional strategies and proposals for a local fieldworker. Over half the women responding to an evaluation questionnaire reported taking specific action to make their own farm safer. ABSTRACT:All faculty members agree that their medical school has an academic purpose and a research purpose, but few medical faculty members agree that a medical education has a social purpose. This is one of the main reasons why there is a serious disjunction between rural societal needs and doctors' education. Rural student clubs are part of a rural counterculture within the orthodoxy of the medical school. Members of these clubs are thus potential agents of curricular change in the field of rural medicine. They are also one of the most potentially influential forces in reminding and helping medical schools to fulfil their social responsibility to the populations of rural Australia. Most rural student clubs are multidisciplinary, and the principles underlying their role in changing the direction and ethos of medical schools also apply to schools of nursing and health sciences.

ABSTRACT:‘Breast is best and bottle is a good second best’ has been a common infant nutrition axiom in Australia in recent times. The impact of the widespread recognition of breastfeeding as the most appropriate form of infant nutrition for the first six months of life has been softened by this almost favourable premise about the suitability of artifical feeding. The promotion of breastfeeding becomes that much more difficult if bottle feeding is seen to be, and promoted as, a comparable alternative to breastfeeding by health professionals and the broader community. There is accumulating evidence to suggest that this is not the case, and that infant feeding with artificial formulas increases the risk of short‐ and long‐term morbidity and mortality. ABSTRACT:Rural general practitioners (GPs) are currently involved in two concurrent and systematic programs of research/evaluation. These programs are generated, respectively, by initiatives in rural health and in general practice. This paper examines the consequences of this involvement by GPs as implementers and/or targets of components of each of these programs, and concludes that it is appropriate that they be provided in a systematic way with the knowledge and skills base they need to empower them to participate in (both developmentally and at the implementation level) and/or to appraise the research impinging on their lives. ABSTRACT:The Ballarat Health Education Forum, has the express purpose of identifying and responding to the educational needs of registered nurses (RNs) in western Victoria. Since the senior nurse members of the forum believed that there was a strong need for management education amongst the nurses involved in middle management in the target region, the total nursing population was surveyed to determine if that perception was shared by RNs themselves. Results of the study confirm that educative programs in management are viewed as a high priority for the professional development of nurses but that family and work commitments are dominant factors which hinder participation. It is argued that education for the managerial role into which many nurses are thrust is sporadic and bereft of appropriate content. Background to the issue of management skills and training among RNs is provided and survey methodology and outcomes are described. The paper discusses the implications of survey outcomes and makes recommendations based on findings.

ABSTRACT:The findings of this study indicate that a large number of nurses practising in the rural areas of Western Australia are regularly working with the elderly. Registered nurses who received some form of education specific to working with the elderly were interested in continuing their education in gerontology. Those registered nurses who did not work regularly with the elderly were the least interested in an external study course. This may be became some nurses working in rural hospitals may prefer to work in their area of expertise rather than in other nursing areas.

ABSTRACT:This study identifies stress‐producing factors reported by registered nurses (RNs) attending serious incidents in an Australian rural hospital's emergency room. A Delphi approach was used to determine the six most significant stress‐producing factors, and these factors were then ranked by two groups of nurses. The rankings were compared using the Spearman Rank Order Coefficient of Correlation and were found to be dependent on years of nursing experience of the RNs taking part in the study. The more experienced group nominated ‘anxiety about nursing a family member or close friend’ as the most significant stress‐producing factor, while the less experienced group reported ‘concerns about nursing a child’ as the most important stress‐producing factor.
