摘要:
&NA;The article presents a rationale for and components of a developmental systems model to guide early intervention programs for vulnerable children and their families. Core principles emphasizing a developmental framework, integration, and inclusion, as well as related principles, are expressed in the context of a multicomponent interactive system. A central organizing feature of the model is its focus on maximizing family patterns of interaction by addressing potential stressors related to risk and disability conditions. It discusses the importance of consistency across the model's conceptual framework, knowledge based on developmental and intervention science, and professional practices and experience.

摘要:
&NA;The article describes BEGINNINGS, an interim Individualized Family Service Plan (IFSP) for use in the neonatal intensive care unit (NICU). It discusses the rationale that supports the process, a description of the format, and recommendations for its implementation. BEGINNINGS initiates early intervention services for infants and their families while infants are still in the NICU. In addition, BEGINNINGS can help identify supports for families as soon as their eligibility is determined and facilitate their transition to community services after the infant is discharged from the hospital. Finally, BEGINNINGS focuses on the developmental needs of newborns and is designed for use by NICU personnel, community service coordinators, and families of infants with special needs. As such, it conforms with the demands of busy intensive care nurseries while it adapts the requirements for an IFSP to the special developmental needs of newborns.

摘要:
&NA;Developmental disabilities associated with a variety of motor, cognitive, language, and social barriers may limit effective, timely communication of pain. Thus infants and young children with disabilities may be highly vulnerable to unrecognized and poorly treated pain. At the very least, pain should be expected to be at least as common if not more frequent a part of the life of a child with a disability, thus making its recognition and management urgent tasks. The assessment and management of pain are challenging and require a multidisciplinary approach that combines a number of techniques. Future efforts need to be directed toward increasing professional awareness about pain, establishing practice guidelines, and undertaking applied and basic research in this field.

摘要:
&NA;A three‐dimensional model is used to compare and contrast various approaches to natural environment interventions and to illustrate similarities and differences in these kinds of practices. The model distinguishes between practices that are contextually or noncontextually based, adult or child directed, and practitioner or nonpractitioner implemented. Our review found that one approach to natural learning environments interventions (practitioner implemented, adult directed) has increasingly been emphasized to the exclusion of other kinds of naturally occurring learning opportunities. Research on what constitutes an optimal mix of contextually based, child‐initiated, and adult‐directed learning opportunities provided by parents and mediated by practitioners is needed to understand what kinds of natural learning opportunities are best suited to optimizing child development outcomes.

摘要:
&NA;Mental health practitioners working with infants, young children, and their families are assigning increasing importance to enhancing early child‐parent relationships. Enhancing early child‐parent relationships is viewed both as a goal in and of itself and as a means to enhancing children's development as a whole. In the past three decades, attachment theory and research have made substantial contributions to understanding early child‐parent relationships. This article discusses the implications of one branch of attachment theory and research, that concerning adult attachment, for enhancing early child‐parent relationships. It begins with a synopsis of the theory and research concerning adult attachment. It then presents the implications of this theory and research for interventions designed to enhance early child‐parent relationships. The article argues that enhancing early child‐parent relationships involves two principal tasks: (a) helping parents identify their children's needs and parents' own responses to these needs; and (b) helping parents gain insight into how their “states of mind with respect to attachment” influence their parenting behaviors and their children's development.

摘要:
&NA;Continuous quality improvement efforts are vital to high‐quality services to infants, toddlers, and their families. This article describes an ongoing quality improvement project within an infant mental health‐based Early Head Start program. Both strategies and challenges in implementation issues and lessons learned in the initial 2‐year phase of the quality improvement project are discussed.
