The acute respiratory distress syndrome regrettably still takes the lives of many individuals each year worldwide. Many of these individuals were in good health before they became at-risk for the acute respiratory distress syndrome and then for unknown reasons developed this highly fatal complication. Critical care clinicians and scientists have admirably risen to the need by seeking ways to improve the outcome in patients with acute respiratory distress syndrome. This brief article identifies and reviews some of the challenges faced by those who strive to understand and reduce the morbidity and mortality of this perplexing disorder.

Mortality rates of the acute respiratory distress syndrome have been high since the syndrome was first described in 1967. The survival rate approximates 40% in all major series. Approximately 90% of all deaths in patients with acute respiratory distress syndrome occur within 2 to 3 weeks after the onset of the syndrome. The major determinant of outcome in acute respiratory distress syndrome is the failure of other vital organ systems. In the past two decades, although we have increased our knowledge about some of the pathophysiological aspects of acute respiratory distress syndrome, improvements in supportive care in ICUs reportedly have not led to an increase in the very low survival rates. Differences in patient selection, severity of underlying disease, patient age, and predisposition for acute respiratory distress syndrome may in fact explain the differences in mortality rates reported in most patient series. Recently, there is some evidence showing that the survival rate is increasing in centers with expertise in the management of acute respiratory distress syndrome.
