SUMMARYThe objective of this paper is to describe the basic physical principles important in magnetic resonance angiography (MRA). The data used were obtained from recent articles on MRA retrieved from Index Medicus 1985–92 and direct experience working with prototype MRA sequences. SUMMARYThe objective of this paper is to describe clinical applications of magnetic resonance angiography. The data used were obtained from recent articles on vascular and flow magnetic resonance imaging retrieved from Index Medicus 1988–91. Other contributions were from the 29th Annual Meeting of the American Society of Neuroradiology held in Washington DC in June 1991 and the 10th Annual Conference of the Society of Magnetic Resonance in Medicine in August 1991. SUMMARYSix patients presenting with supradiaphragmatic Hodgkin's disease are presented to demonstrate the potential benefits of chest computed tomography (CT) scanning as a routine staging procedure. These cases show that CT scanning can detect mediastinal and lung involvement not readily detected by other investigations, and that such findings can influence the radiotherapy plan, the need for extended radiotherapy fields or the use of chemotherapy. Following treatment, CT scanning can be useful to assess treatment response and may permit earlier detection of relapse.The use of chest CT scanning as a routine staging procedure in all patients with supradiaphragmatic Hodgkin's disease is advocated.

SUMMARYAn observational study was undertaken to obtain measures of the impact of magnetic resonance imaging (MRI) on diagnosis, patient management and patient outcome. A minimum data set at the time of examination was used for 1119 consecutive patients referred by specialists for MRI of the brain or spine. Three month follow up of 707 brain examinations and 235 spinal examinations was undertaken using a questionnaire on diagnosis and patient management.Magnetic resonance imaging made a dominant contribution to final diagnoses of neoplasia and vascular disorders, but was less significant for white matter disease. In a high proportion of cases other types of examination also influenced the final diagnosis. Magnetic resonance imaging affected patient management in a high proportion of spinal examinations and in cases of cerebral neoplasm, with lesser contributions to cases of cerebrovascular disorder and white matter disease. While MRI was considered superior to other imaging methods, which it could often replace, in practice it will form only one input to the diagnostic decision.

SUMMARYThis complex study was designed to measure the transport and excretion characteristics of gadolinium ethoxybenzyl diethylenetriaminepentacetic acid (Gd‐EOB‐DTPA) in dog's livers following bolus and infusion. Simultaneous T1 magnetic resonance imaging was performed to measure maximum signal enhancement.Anaesthetized dogs had cannulation of the common bile duct and urinary bladder for collections and cannulation of the femoral artery and vein for monitoring, blood sampling and infusion. Gd‐EOB‐DTPA was administered by bolus (range 12.5–200 μmol/ kg) and infusion (range 0.4–6.4 μmol/min per kg).An hepatic transport maximum 0.09–0.15 μmol/min/kg was achieved with a blood concentration of 0.03–0.06 μmol/mL. Marked hepatic affinity for Gd‐EOB‐DTPA was demonstrated with measurements of liver concentration. Maximum T1 signal enhancement was achieved with blood Gd‐EOB‐DTPA concentration of 0.02–0.03 μmol/mL and a liver concentration of 1–2 μmol/g.The transport maximum for Gd‐EOB‐DTPA in the dog was similar to that for ipodate and iodipamide and effective imaging was achieved with sub‐maximal doses. SUMMARYThree cases of clival chordomas are reviewed and the findings are compared to those in the recent literature. In the first case the tumour arose from the basion, spreading downwards to invade the upper cervical canal and the paravertebral fascial planes. The second case is a rare case since it affects a 12 year old pre‐adolescent boy. The third case showed the characteristic features of petro‐clival erosion and a large extracranial component in the nasopharynx.In all three cases, computed tomography was an excellent modality for demonstrating bone destruction, sequestra and calcification but inferior to magnetic resonance imaging (MRI) in demonstrating the soft tissue extent of the tumour. The multiplanar capability of MRI was found to be particularly useful for planning treatment. Vertebral angiography can demonstrate the tumours by vessel displacement, encasement and vascular staining. Two cases had angiography and demonstrated tumour blush. SUMMARYThe chest radiographs and lung function tests of 41 patients attending the cystic fibrosis clinic at Royal Prince Alfred Hospital were reviewed. The chest radiographs were scored using three different scoring systems: the Shwachman and Kulczycki system (as modified by Doershuk), the National Institute of Health (NIH) system described by Taussig in 1973 and the Brasfield system. The scores were correlated with lung function tests. Significant correlations were found between the radiological scores and the respiratory variables; the best correlation was with the forced expiratory volume in one second (FEV1% predicted). All three scoring systems showed a high degree of reproducibility of scores when a second radiologist was asked to score the same radiographs independently. The difference in scores between the radiologists was not significant for the NIH and the Brasfield systems. The Brasfield system is, however, the system of choice because it allows the assessment of all the major pathological features seen in cystic fibrotic chest films and consistently has the best agreement with all the lung function variables. It was also found that radiological evidence of lung hyperinflation may not be a good indicator of disease progression.

SUMMARYA middle aged woman with a pyrexia of unknown origin was shown to have borderline lepromatous leprosy. Early gallium‐67 scintigraphy demonstrated increased uptake in the subcutaneous tissues of the face and thighs.
