Summary27 cases of primary malignant and benign rib tumours are reported. The most common malignant rib tumour in childhood is Ewing sarcoma (20 cases). Some other rare bone tumours and tumorous conditions (reticulosarcoma, aneurysmal bone cyst, monostotic rib eosinophilic granuloma, osteoid osteoma and lymphangioma) are also described.In the authors' opinion Ewing sarcoma presents with characteristic clinico‐radiographic findings in most of the cases. ABSTRACTBone metastases may result from arterial, retrograde venous or direct spread. Three groups of patients; with carcinoma of lung, carcinoma of breast and carcinoma of prostate, each of approximately 50 individuals were studied to examine patterns of spread in order to relate this to the mechanism of seeding.Statistically significant differences (p<0.05) were observed in early bone spread in carcinoma of prostate (absence of skull involvement), carcinoma of lung (presences of posterior chest wall involvement) and carcinoma of breast (presence of anterior chest wall involvement).The results support retrograde venous reflux as being an important contributing mode of spread for carcinoma of prostate and local spread as being relevant in the early spread of carcinoma of lung and breast.

ABSTRACTThe painful degenerate disc is a recognised cause of low back pain. Magnetic Resonance Imaging (MRI) has now replaced discography in the non‐invasive assessment of disc degeneration. However, the prohibitive capital expense of MRI and the small number of MR units in Australia produce limitations in clinical access. In contrast. Computed Tomography(CT)is readily available and is performed in most patients prior to MRI referral.This prospective study was undertaken to determine whether preliminary CT could offer any information about disc degeneration and so reduce the demand on a MRI scanner.30 consecutive patients were studied all of whom had both CT and MRI examinations. Of a total 107 discs examined by both techniques, MRI was able to identify 37 degenerate discs. Conclusive evidence of degeneration (i.e. the presence of intervertebral gas) was only seen in 3 discs at CT (1 patient). Of the 29 posterior disc bulges found on CT, all were both bulging and degenerate on MRI.Indications for MRI based on the CT findings are recommended. Using these criteria, 13% (4 patients) of this study group could have avoided an expensive and unnecessary MR investigation. ABSTRACTTechnetium‐99m planar sulfur colloid scintigraphy, red blood cell imaging and ultrasonography were performed in eighteen patients of hepatic hemangioma to evaluate their diagnostic utility. Radiocolloid scans detected fourteen hemangiomas in 12 patients as focal hepatic defects but missed 9 lesions smaller than 2.5cm in the other 6 patients, accounting for a 61% sensitivity. All 23 lesions were avascular or normovascular on radionuclide flow study. Serial planar red blood cell scintigraphy showed characteristic gradually increasing focal isotopic accumulation in the 14 hemangiomas which had identifiable cold areas on the radiocolloid study. One of 6 patients with normal radiocolloid scan also showed the typical increasing blood pool pattern of hemangioma, five had normal planar scans till 11/2 hours. SPECT red blood cell imaging in these 5 patients detected an additional seven hemangiomas. All 7 lesions were smaller than 2.5cm. One 1.5cm hemangioma was missed on both planar and SPECT red blood cell imaging. Region of interest ratios in SPECT transaxial images confirmed gradually increasing concentration of labeled red blood cells in the lesions. Ultrasonography detected 22 of the 23 hemangiomas. Ten were hyperechoic, average size 2.25cm. Twelve were of mixed pattern, generally larger, average size 7.2cm. Sonography is useful in the detection of hepatic hemangiomas as a focal mass lesion, but the variable sonographic appearance precludes a definite pathological diagnosis. Technetium‐99m red blood cell imaging appears a sensitive technique in detection of hepatic hemangioma, with SPECT being especially useful in locating smaller lesions. The addition of SPECT to planar red blood cell imaging considerably improves the sensitivity of this highly specific technique for an accurate non‐invasive diagnosis in hepatic heman

ABSTRACTTwelve cases of obstructive jaundice in whom ultrasound failed to demonstrate the site and/or the cause of obstruction of the biliary tract were examined with magnetic resonance imaging (MRI), correctly diagnosing the site and cause of obstruction in 10 of 12 surgically proven cases. In one case of cholangiocarcinoma, the site of obstruction was well shown on MR but a definite cause could not be ascertained. In another patient who developed intermittent jaundice following surgery for choledochal cyst, MR demonstrated a solitary stone in the common hepatic duct. Surgical confirmation could not be achieved as the patient was lost to follow up. There were 6 cases of choledocholithiasis, 3 cases of gall bladder carcinoma and one case each of pancreatic adenocarcinoma and cholangiocarcinoma. It is believed that MRI will provide obstructive jaundice and will be able to minimize the use of percutaneous transhepatic cholangiography (PTC) and endoscopic retrograde cholangiopancreatography (ERCP) in view of its ability to perform multiplanar imaging in multiple sequences.

ABSTRACTEleven patients who had radiological procedures because of urinary leakage or ureteric obstruction in transplant kidneys are reviewed in order to judge the clinical efficacy of interventional radiological techniques in the management of these complications. Initial technical success was attained in 9/9 dilatation attempts and 17/21 (81%) stent placement attempts. It is thought that 9/11 (82%) patients benefited from the radiological attempts at treatment and at long term follow up 7 kidneys were still functioning, 3 failed due to rejection and one after failed renal artery surgery.The complication rate due to the radiological procedure was low, with one iatrogenic urine leak being the single major procedural complication. Two patients had urinary tract infections while stented and there were two patients with perirenal abscesses.

ABSTRACTEight DSA units in the Sydney region were evaluated by a set of simple performance tests.Low contrast resolution and iodine sensitivity tests exhibited the most variation between the six fully tested units.A range of performance levels was measured.
