&NA;The tragic events of September 11, 2001, not only altered the famous skyline of New York City, but also changed forever the hearts and minds of all Americans. What was unthinkable just a year ago is now a reality. The world is accepting the fact that there are those who are willing to use weapons of mass destruction against civilian targets with the desire to extract the highest possible casualties. The goal of this article is to provide the reader with the history and background of manmade disasters as an introduction to this issue devoted to disaster medicine.

&NA;September 11, 2001, will be indelibly impressed on the hearts and minds of all Americans. Now, America must be prepared for all conceivable terrorist attacks in order to ensure survival of our country and our interests throughout the world. Biologic terrorism could produce thousands of American casualties if appropriate preparation and training are not undertaken. This article discusses some of the commonly anticipated forms of bioterrorism and methods that may be used to treat and manage victims and potential victims of biologic terrorism. Health care workers must be thoroughly familiar with the major anticipated biologic agents such as anthrax, smallpox, botulism, plague, ebola, and staph. Hospitals and other health care facilities must have written plans to assist staff in the handling of biologic casualties.

&NA;Chemical warfare agents represent a potential terrorist threat against civilian populations. These agents can be easily synthesized and, if dispersed properly, could lead to mass casualties. It is important for the emergency department physician to be familiar with the various agents, to know how to decontaminate exposed patients, and to be able to treat individual exposures. This article presents the four major types of chemical agents: pulmonary toxic, nerve, blood, and blistering.

&NA;Radioactive agents pose a significant threat to the efficient operation of any emergency department and challenge the ability of emergency physicians to provide lifesaving patient care. A well‐planned, organized system of triage and hospital preparedness is critical. All health care providers involved in the early treatment of these patients must understand the clinical presentations, treatment options, and preventive measures to prevent further contamination of other individuals. This article reviews some of the important concepts in management of nuclear catastrophes.

&NA;The worst terrorist attack in the history of the United States occurred on September 11, 2001, permanently changing the lives of Americans, uniting the country, and prompting the need for terrorist disaster preparedness. The tragic, catastrophic explosions of hijacked, passenger‐filled commercial aircraft into the World Trade Center towers in New York City, into the Pentagon in Washington, DC, and a horrific field crash in Pennsylvania illustrate the magnitude of injuries and death that can result from a traumatic jet fuel blast explosion. Emergency physicians need to be familiar with explosion‐related trauma as part of domestic preparedness.

&NA;The delivery of medical care in response to a disaster differs radically from the routine experience of health care providers. In order to ensure the greatest number of potential survivors, medical resources are diverted from those who will recover, or are likely to die, irrespective of the medical care they receive, and concentrated on those critically ill casualties with a reasonable probability of survival. Triage, a means of sorting casualties to determine the priority of medical treatment, is the cornerstone in the medical management of a disaster situation.

&NA;The use of biologic weapons has become an important focus of the national goal to become domestically prepared for the new era of terrorist threats against the United States. Biologic agents are of three types: bacteria, viruses, and toxins. This article reviews three important biologic toxins—botulinum, ricin, and T‐2 mycotoxin—that have the potential to be used as agents of biologic warfare and as weapons of mass destruction.
