摘要:
Gastric decontamination following toxic ingestions or drug overdose can no longer be justified routinely, but remains useful in selected cases. Likewise, the options now include not only induced emesis or lavage, but also activated charcoal in single or repeat doses, cathartics, and whole bowel irrigation. The choice of one or more of these treatments must be made with knowledge of their efficacy and limitations. This article reviews these techniques, and advocates each for use in selected circumstances.

A recent number of hospital-based research studies have shown that emergency physicians do an inadequate job of recognizing potential drug interactions. There are a number of common pharmacokinetic drug interactions that can result in elevated drug levels which may produce secondary patient toxicity. Therefore, it is extremely important that emergency physicians gain a greater understanding of pharmacokinetic principles. This article reviews the essential pharmacokinetic principles that will allow emergency physicians to recognize and prevent potential drug interactions and thus prevent iatrogenic patient poisoning.

Flumazenil is a specific benzodiazepine antagonist that is useful in reversing the sedation and respiratory depression often associated with benzodiazepine administration or intentional overdosage. The selective use of this drug may provide useful diagnostic information and may eliminate the need for mechanical ventilation or other invasive supportive measures. Although controversy exists regarding the induction of seizure activity if flumazenil is used in the setting of mixed cyclic antidepressant-benzodiazepine overdose, worldwide experience with flumazenil has conclusively validated its safety and efficacy.

Trazodone is a commonly prescribed antidepressant medication that is frequently encountered in drug overdoses. All too often, emergency physicians are uncertain how to treat patients who have ingested trazodone. Unfortunately, this uncertainty leads to both patient under treatment and over treatment. This article discusses trazodone pharmacology and toxicology and presents management principles that will allow emergency physicians to appropriately treat patients with trazodone ingestions.

Mind influencing drugs have been with us since the prehistoric period. Recently, there has been a marked increase in the number of prescribed and illicit psychotropic drugs. This has resulted in an increase in the number of patients presenting to the emergency department with toxicity from psychotherapeutic agents. A familiarity with modern concepts of toxicology management is essential. The pharmacology, side-effects of various psychotherapeutic agents as well as clinical presentation and management of the resultant intoxications are discussed.

Serotonin syndrome represents a recently described complication of drug therapy with medication(s) that increase serotonin neurotransmission. This syndrome consists of alterations in cognition, automatic nervous system function, and neuromuscular function. It is rarely fatal and usually responds to supportive care alone. This article presents two illustrative cases of serotonin syndrome and reviews the known medical literature on this topic. In addition, it also discusses serotonin pharmacology, physiology, and its receptor classification.
