&NA;Disorders of the Achilles tendon complex are common. The typical patient is a male in his fourth decade of life who participates in recreational sports activities. Many factors have been implicated that predispose to Achilles tendon problems including blood supply, age, sex, and activity level. Injuries may be acute or chronic. Chronic injuries are treated conservatively. Treatment of acute injuries begins in the emergency department with an accurate diagnosis. Successful definitive treatment of acute injuries is patient specific and may be conservative or operative.

&NA;Venous and arterial thrombosis are major contributors to morbidity and mortality in the United States. Individuals with a predisposition to thrombosis are at especially great risk. Laboratory evaluation should be promoted by the occurrence of thrombosis in young patients, i.e. less than 60 years old, even when the clot appears in the setting of a predisposition, recurrent thrombosis, or thrombosis in an unusual anatomic location. The presence of thrombosis in more than a single family member steers one toward hereditary causes. Commonly, thrombotic tendencies are the result of a combination of more than a single hereditary cause, the sum of multiple, hereditary and acquired abnormalities or solely acquired entities.

&NA;Physicians have long been interested in limb‐lengthening procedures. It was not until the 1950s and 1960s, however, that a better understanding of the biology of lengthening emerged from the work of Ilizarov and others, which led to a reduction in the morbidity associated with these procedures, Despite this progress, complications such as infection, contracture, premature consolidation, and nerve and vascular injury still occur and must be recognized and resolved early in order to accomplish the goals of lengthening. This article reviews the concepts of limb lengthening and its complications in order to improve recognition and treatment in the emergency department.

&NA;The most common reason for performing a lower extremity amputation in the United States is treatment of complications of systemic diseases, specifically diabetes mellitus and peripheral vascular disease. As a consequence, the most commonly encountered, post‐operative problems are wound‐related hematoma, infection, and wound breakdown. The amputee may experience severe pain related to progressive ischemia, phantom sensations, and emotional distress, In evaluating the recent amputee with pain, swelling, and/or wound problems, both systemic and local factors must be considered in establishing the diagnosis and formulating a treatment plan.

&NA;Complications of foot and ankle surgery are rare, but often engender significant anxiety. Most foot and ankle surgery is performed in the outpatient setting; therefore, patient presentation to the emergency department with a possible complication is a very real possibility. Most foot and ankle surgery complications can be managed quite simply with a dressing or cast change, consultation with the surgeon, and outpatient followup. Nevertheless, catastrophic complications are possible that may require emergent evaluation and treatment. Complications can be due to wound problems, internal fixation devices, cast or splint problems, compartment syndrome, and neurologic deficits. Removal of any dressing, cast, or splint and a thorough history and physical examination are essential to the management of foot and ankle surgery complications.

&NA;Emergency department thoracotomy is a dramatic and debated procedure performed in the emergency department. In the care of trauma patients, therapeutic indications have narrowed over the past 25 years, and several studies have proven that a select group of individuals benefits from this procedure. Open cardiac massage for nontraumatic cardiac arrest was used extensively in the past and has been shown to improve the hemodynamic status of these patients when compared with closed cardiopulmonary resuscitation. This article reviews both of these topics and examines the risk/benefit of performing an emergency department thoracotomy.

&NA;Pralidoxime is an effective biochemical antidote for organophosphate overdose and toxicity. Its effects at both the nicotinic and muscarinic receptor levels prevent both short‐term and long‐term sequelae of the toxicity. In this article, we report on a case series of transient blindness with the administration of pralidoxime for organophosphate toxicity.
