摘要:
The treatment of pediatric patients in the out-of-hospital setting may be an anxiety-provoking experience. This is due in part to limited pediatric training opportunities as well as the somewhat rare opportunity to care for actual patients. The article reviews the finer points of care of the pediatric patient in the out-of-hospital setting. Topics discussed include the history of emergency medical services (EMS) for children, training, EMS access, scene control, history acquisition, physical examination, common presentations and treatment, transportation, documentation, interfacility transports, consent, stress, and prevention. Out-of-hospital emergency medical care providers must be prepared to care for any patient who accesses their EMS system regardless of the patient's age or chief complaint. EMS for children must never be a standalone entity; rather, it must be fully integrated into every EMS system.

Cases of pediatric arrest can elicit a strong emotional response from the most seasoned emergency care provider. The structured, prioritized approach to airway, breathing, and circulation (ABC) provides not only the best chance of survival for the patient but also a viable coping mechanism for the care provider. Likewise, numerous non-cardiorespiratory arrest situations exist where close attention to the ABC algorithms will result in decreased morbidity and mortality. This is the foundation upon which emergency medicine functions.

The most commonly performed procedures on children include those for vascular access (for both diagnostic and therapeutic purposes) and those to obtain body fluids (venous and arterial blood, cerebrospinal fluid, and urine) for diagnostic evaluation. The article reviews the indications, contraindications, and complications of peripheral and central vascular access, umbilical vessel catheterization, intraosseous access, arterial and lumber punctures, and suprapubic aspiration of the urinary bladder. Suggestions to aid in their accomplishment are also provided.

Pediatric suicidality is growing in prevalence and serves as the paradigm for guidance of emergency care professionals in approaching all pediatric psychiatric emergencies. The identification of children and adolescents at risk for suicide is a most important function. The physical environment of the emergency department, combined with the stress of rapid assessment and intervention, makes pediatric psychiatric emergencies challenging.

Pediatric patients account for the overwhelming majority of US poisonings as reported by the American Association of Poison Control Centers. Infants and toddlers are most prone to poisoning as a result of their unique developmental, behavioral, and environmental characteristics. Although most childhood poisonings are clinically insignificant, a substantial number of life-threatening and fatal poisonings occur annually. Accessibility of toxic substances is the most important modifiable risk factor in pediatric poisonings. The article reviews the clinical diagnosis of poisoning, decontamination issues, antidotes, and management.

The article presents a general discussion of functional bone anatomy, fracture types, and fracture classifications and then reviews current concepts of pediatric orthopedic emergencies. Special consideration is given to common nontraumatic emergencies. General out-of-hospital features are noted, as are emergency management issues that apply to each specific fracture, dislocation, or nontraumatic orthopedic emergency. Orthopedic manifestations of child abuse, with emphasis on when to consider child abuse as the etiology of a pediatric orthopedic injury, are considered.
