Magnetic resonance imaging (MRI) has provided a totally new approach to evaluating the musculoskeletal soft tissues; it demonstrates many structures that were relatively invisible to established imaging techniques. MRI can show vascular and articular structures that could previously be seen only after injection of artificial contrast media. This modality demonstrates both physiologic and anatomic information and provides data about the fluid content and solid nature of lesions. Although MRI is not usually tissue specific, a tissue diagnosis may sometimes be suggested. The inherent image contrast exceeds all other imaging modalities, while the spatial resolution approaches that of computed tomography (CT). MRI not only provides high intertissue contrast, but also, by varying the sequence parameters, has the ability to alter the relative contrast between different tissues. All of these advantages are achieved without the need for ionizing radiation and without known health hazards in most patients. MRI is rapidly establishing itself as the definitive imaging method for the soft tissues.1–5

Is a few short years, magnetic resonance imaging (MRI) has established for itself an increasingly prominent role in the evaluation of the solitary tumor or tumorlike lesion of bone.1–7This new technique now either complements or competes with preexisting diagnostic modalities that include radiography, conventional tomography, scintigraphy, computed tomography (CT), and arteriography. The authors' experience accrued from the evaluation of approximately 130 patients forms the basis of this article. MRI will be discussed with each of the other modalities in the context of their respective clinical roles. Conclusions can thereby be drawn for the appropriate use of MRI and can obviate redundant imaging procedures.

Magnetic resonance imaging (MRI) has established itself as a valuable tool for bone marrow imaging.1–3It has proven to be the most sensitive technique available for imaging a variety of bone marrow disorders, in many cases with sensitivity comparable or superior to nuclear medicine studies and resolution rivaling computed tomography (CT).The sensitivity of MRI to changes in bone marrow is due to the inherent properties of MRI. Signal intensity in MR images largely results from the combination of relaxation properties and the concentration and chemical form of mobile protons. Perhaps in no other organ of the body is MR signal intensity so profoundly influenced by the relative concentrations of fat and water as in the bone marrow. Paramagnetic and superparamagnetic effects are also important in the genesis of MR signal intensity changes, as the bone marrow is critically involved in storage and processing of iron.

Injury-related imaging has emerged as one of the more commonly used and enthusiastically accepted areas of musculoskeletal magnetic resonance imaging (MRI). The multiplanar capability of the technique coupled with its soft tissue contrast resolution make MRI superbly suited for evaluation of the principal constituents of the musculoskeletal system. The noninvasive nature of MRI has facilitated evaluation of patients who have experienced recent trauma. MRI has demonstrated an expanded diagnostic capacity that has allowed depiction of a broad spectrum of trauma-related abnormalities, many of which have previously escaped early detection. The sensitivity and accuracy of the method have allowed MRI to challenge and, in an increasing number of situations, replace previously established and often more invasive diagnostic procedures.This article reviews the principal applications of MRI in the assessment of injuries to the musculoskeletal system. The emphasis is on the most commonly encountered conditions, both accidental and sports related. A consideration of technical factors as they pertain to the design and interpretation of trauma-related MRI is presented, followed by a regional (anatomic) approach to specific MRI applications.
