This report deals with the question of the susceptibility for IDD association with the recently described HLA‐linked SB system. The SB system is located centromeric of HLA‐DR between HLA‐DR and GLO. At present five specificities of the SB system, which behave as alleles, can be recognized. A total of 40 IDD patients and 96 normal controls were characterized for HLA‐A, ‐B, ‐C, ‐DR and SB antigens. Our results confirmed the strong positive association of IDD with HLA‐DR3 and ‐DR4 and the negative association of IDD with HLA‐DR2.The genetic analysis of the SB system and IDD, however, demonstrated no significant association between alleles of SB and susceptibility for IDD. The analysis of association between alleles of HLA‐DR and SB revealed no significant linkage disequilibrium in IDD patients and a significant linkage disequilibrium between SB1 and HLA‐DR3 in the controls. The survival data of 151 consecutive cadaver kidneys transplanted by the two transplantation centres of Copenhagen from September 1, 1980 to September 30, 1982 with an observation period of at least 3 months have been analyzed. The HLA‐DR types could not be established in 4 out of 130 donors. In most of the analyses only the well‐defined antigens DR1, 2, 3, 4, 5, 7, and w8 were included. Inclusions of the DRw6, w9, and w10 antigens in the matching did not change the results. The one‐year graft survival (GS) was 72.1% for 97 HLA‐DR compatible kidneys as compared to 41.4% for 49 incompatible kidneys (p = .0007); this difference remained highly significant when stratified for the recipient status of pretransfusion, risk, and age. Non‐transfused recipients had a fairly good GS but had received significantly better matched kidneys than the remaining recipients. The transfusion “effect” became barely significant (p = .054) when stratified for DR. There was no significant influence of donor or recipient DRw6‐type as defined in this study. The GS was not significantly influenced by HLA‐A, B matching, recipient antibody status, B cell cross‐matches, transplant, or donor centre. Special efforts were made to assign the HLA‐DR phenotypes of the recipients and donors as accurately as possible. In 39 cases, there was a discrepancy between the result of the acute DR‐typing of the donor and the final result based on subsequent typings. The acute DR match had no influence on GS in these recipients whereas the final match had a significant influence in the same group, which in a way comprise a randomized trial. In the total material, the acute DR match still showed an influence on GS, but the significance decreased by a factor 20. This illustrates how the quality of DR typing may influence the results of the analyses. HLA‐A and ‐B antigens incidence were investigated in 100 cases of Seronegative Spondyloarthropathies and in 303 control individuals of a Venezuelan Mestizo population. Significative increased incidence of B27 was found among 69% of patients with ankylosing spondylitis and 68% with Reiter syndrome being the incidence in the control group of 2.9%. The B27 negative patients showed 40.4% positivity for Bw35, with spondylitic lesion in 12 out of 17 cases. Our results question the applicability of B27 as diagnostic criteria in the Mestizo patients bearers of a Seronegative Spondyloarthropathy and suggest the need to investigate further the role of antigens of the B locus in the pathophysiology of these clinical entit

Two previous surveys of ankylosing spondylitis (AS) in Mexican Mestizos found HLA‐B27 frequencies of 68.6% and 78% and a relative risk (RR) of 37.05 and 120.88, respectively. We examined an additional group of Mexican Mestizos with AS and found an HLA‐B27 frequency of 80.77% and a RR of 99.24. Sera from 464 multiparous women of mixed ethnic origin and 114 Amerindian mothers (Warao of Western Venezuela), were studied for lymphocytotoxicity using a panel of B and T lymphocytes isolated from 50 to 100 individuals from the same populations. Twenty‐five and 9.65% of the sera, respectively, showed activity against at least 5% of the panel.
