The main purpose of Eurotransplant, an international organ exchange organisation, is to maximize the survival time of transplanted organs through tissue typing and matching. Regular proficiency testings are necessary to assess the repro‐ducibility and the reliability of the HLA typings performed by the individual typing centres.Two different protocols have been used: 1) since 1978, 35 samples of blood and donor spleen were used for cell exchanges (CE) and typed by all participating laboratories; 2) since 1977 samples of donor spleen have been shipped to the reference laboratory in Leiden for retyping (RD) resulting in over 2600 comparable HLA typings.The disrepancy rates of the HLA‐A, B, C and DR specificities were analysed over the years. A large number of discrepancies was due to false negative assignments which led to assumed homozygosity at one or more HLA‐loci. The recognition of HLA‐DR has improved dramatically but with much variation per DR specificity. The concordancy rates for HLA‐AB + DR have improved from 40% in 1981 to 91% for CE and 80% for RD typings in 1984. The present study was designed to examine the possible association of HLA antigens and schistosomal hepatosplenomegaly and colonic polyposis in a large number of patients and controls. The results show that hepatosplenomegaly was significantly associated with the presence of two HLA antigens: HLA‐Al and B5; and colonic polyposis to HLA‐B5 and B8. Furthermore, asymptomatic infection was significantly seen more in subjects with CW2. Treatment of human peripheral blood mononuclear cells with UVB light, the photosensitizing system 8‐methoxypsoralen plus UVA light (PUVA), or hematopor‐phyrin derivative plus visible light leads to an inhibition of their stimulatory capacity in an allogeneic mixed leukocyte reaction despite unaltered expression of HLA‐DR antigens when tested immediately after irradiation. However, HLA‐DR positive cells disappear among mononuclear cells in the interval between 4 and 8 h after treatment with either UVB, PUVA, hematoporphyrin derivative and light, or heating to 45°C. The known cross‐reactivity of monoclonal antibodies prepared against CD1 and HLA‐DR antigens with skin components prompted us to study the reactivity pattern of human eccrine sweat glands with a panel of monoclonal antibodies directed against CD1 antigens (OKT6, BL6, D‐47) and against HLA‐class II antigens (anti‐DR, BL2, LEU‐10, IV‐D12, MAJA‐7). The labelling pattern of eccrine glands with the panel of monoclonal antibodies used in this study permits to establish three different antigenic compartments on eccrine glands: 1) acrosyringium and distal part of dermal duct anti‐DR+, BL2+, LEU‐10+, IV‐D12+; 2) proximal part of dermal duct MAJA‐7+; 3) secretory part D‐47+. The distribution of HLA‐A, ‐B and ‐DR antigens as well as blood groups and secretor status was studied in sporadic, North Indian patients of rheumatic fever and rheumatic heart disease. While HLA‐Aw33 occurred with an increased frequency in the patient group (X2=4.01), no statistically significant differences were observed in the frequency of B‐locus antigens. In the DR locus, HLA‐DR3 was found to be significantly increased (50% vs 26.1%, X2=13.8) and DR2 significantly reduced (21.8% vs 47.0%, X2=15.6). Also, there was a preponderance of non‐‘O’ blood group individuals in the patient group as compared to controls. The DR3 association was significant only in those patients of RHD who did not have any previous history of rheumatic fever. These results indicate that susceptibility to rheumatic heart disease is HLA‐class II mediated, with HLA‐DR3 influencing susceptibility and DR2
