The influence of HLA phenotypes on the response to parenteral gold in rheumatoid arthritis |

One hundred and ten patients with rheumatoid arthritis (RA) were studied for a possible influence of HLA phenotypes on the reaction to parenteral gold in the first 6 months of treatment, in terms of both clinical response and toxicity. Frequencies of HLA‐B8 and ‐DR3 were significantly increased in patients who responded excellently to gold treatment as compared with non‐responders (p = 0.04 for both antigens). On the other hand, for HLA‐DR7 there was a tendency for increased frequency in non‐responders versus excellent and moderate responders (p<0.03; Pc=n.s.). Drug toxicity was higher in excellent than in non‐responders (p = 0.04), being exceptionally high in male excellent responders (85% versus 33% in females, p<0.01), probably due to the increased frequency in B8 and DR3 in the excellent responder group as a whole and in the excellent responder males in particular. We conclude that HLA antigens B8 and DR3 co‐determine both toxicity and excellent clinical response to parenteral gold, whereas the presence of DR7 is possibly associated with non‐response. In an attempt to study the variation of associations between HLA and rheumatoid disease a population of 44 Ashkenazi and 29 non‐Ashkenazi patients with Rheumatoid Arthritis were tested for HLA‐A, B, C and DR antigens and compared with the relevant control groups.In contrast to the results obtained in Middle European or North American Caucasians, Rheumatoid Arthritis in Israel is not associated with B15 and Cw3, indicating that it is very unlikely that B‐ and C‐locus antigens are involved in coding for disease susceptibility for RA.The allele DR4 which is found associated with RA in almost all populations tested so far was in the total patient group (47.9%) slightly but not significantly more frequent than in the control group (38.3%). This difference was entirely due to a non‐significant increase in the frequency of DR4 in the Ashkenazi patients (54.5%) compared to controls (40%), while the frequency of DR4 in non‐Ashkenazi patients and controls was virtually identical (38.0% vs 36.7%).Another surprising finding was that the frequency of HLA‐DR1, which has been reported to be increased in different populations of patients with RA was found to be completely normal in the present study on Israeli patients. The alleles of the Bf and the GLO system did not show any significant difference between patients and controls.The surprising results concerning the lack of association of RA in Israeli patients with DR4 and with DR1 lead to the speculation, that RA may be associated not with DR4 but with a marker of a closely linked locus which is very highly associated in Middle European Caucasians with DR4 but which is less strongly associated with DR4 in Israeli Jews. The 3A33 monoclonal antibody, obtained by fusing rat immune lymphocytes with mouse plasmacytoma cells, was directed against mouse macrophages. Antibody 3A33, a rat IgG2a, reacted with macrophages from all the mouse strains tested, with mouse blood monocytes and with 56% of bone marrow cells, but not with T lymphocytes. It immunoprecipitated an antigen with α and β subunits, found to be identical to Mac‐1 antigen after cross‐absorption experiments with M1/70 monoclonal antibody. The two antigenic determinants of the Mac‐1 molecule identified by the 3A33 and M1/70 antibodies both displayed reduced expression on inflammatory macrophages and comparable resistance to trypsin digestion. The sites of the determinants on this molecule seemed close together judging from the ability of both the 3A33 and M1/70 antibodies to block C3bi receptor sites and compete for cell binding. However, unlike antibody M1/70, 3A33 never reacted with human cells bearing Mac‐1 antigen. We studied the polymorphism of HLA‐DR2 by Southern blot analysis. Genomic DNA from 12 DR2 positive unrelated individuals was digested separately with five restriction endonucleases, Bam HI, Eco RI, Eco RV, Hind III and Pvu II. Hybridizing restriction fragments were visualized using radiolabeled β‐chain cDNA probes homologous to the DR and DQ subregions of the HLA. The results in the present study demonstrate that the two subtypes of DR2, DR2.1 and DR2.2, as defined by serological and cellular (PLT) techniques can be identified by Southern blot analysis. These two subtypes were identifiable after cleaving genomic DNA with Bam HI and EcoRV. DR2.1 correlated with restriction endonuclease fragments at 2.8 kb (DRβ) and 6.6 kb (DQβ) after Bam HI digest, while DR2.2 correlated with a 2.9 kb (DRβ) and a 2.6 kb (DQβ) restriction fragment after Eco RV digest. In addition, inheritance of the restriction fragment lengths defining DR2.1 and DR2.2 were observed in two families. Monoclonal antibodies were used to analyse skin samples from human fetuses (15–19 weeks gestational age) for the presence of class II major histocompatibility antigens (HLA‐DR, DQ and DP). Cells expressing these antigens were clearly demonstrated within the epidermis from a proportion (4/7) of cases and were completely absent from the others. By conjugating a monoclonal IgM antibody of CD8‐specificity to magnetite‐containing polymer particles, we have developed a rapid and simple one‐step procedure for positive selection of T8 cells. The method ensured good yield and viability of pure T8 cells. The positively selected T8‐fraction from MLC activated T cells retained their cytolytic capacity and gave a moderate proliferative response. Most of the proliferative response occurred in the negatively selected T4 population which showed only a marginal cytotoxic activity against PHA blasts as target cells.
