The role of HLA‐A and ‐B and HLA‐D compatibility was analyzed in 63 recipients of intrafamilial kidney grafts. At 1 year post‐transplantation the survival of grafts from HLA‐A and ‐B compatible donors was 89% and that of grafts from HLA‐A and/or ‐B incompatible donors 77%.Recipients with a Relative Response (RR) in MLC of<20% towards their donors, indicating HLA‐D compatibility, had a 4‐year graft survival of 100%. After 1 year the graft survival of this group was statistically different (P60% the 2‐year graft survival was 46%. This was lower than that of patients with RR<20% (P<0.001) and that of patients with an RR of 20–60% (P<0.01). The recipients with an RR of 20–60%, indicating slight HLA‐D incompatibility with their donors, had a 2‐year graft survival of 88%.We concluded that graft survival was correlated to the magnitude of MLC response between recipients and donors in such a way that a high MLC response indicating HLA‐D incompatibility was associated with a high frequency of graft rejection while a low MLC response was associated with graft survival. The presence of immune complexes or aggregated IgG in the microlymphocytotoxicity assay may cause inhibition of cytoxity. When HLA‐A and ‐B antigens are the subject of the test the effect is minimal; however, when B cell alloantigens are being examined the effect is marked. The genetics of five HLA—D specificities (Dw1, Dw2, Dw3, Dw4 and Dw6) have been assessed in 21 normal families with four or more children. The HLA—D traits, as defined by typing response against homozygous typing cells, normally behave as dominant characters. The data support the concept of allelic factors. The logical flaw in the basic algorithm of MLC typing (HLA—D typing), i.e. to draw positive conclusions from negative observations, has been amply reinforced in the following studies. Five assignments could not be verified genetically under the assumption of dominant traits. The search for HLA association in spina bifida is particularly interesting since this condition can be associated with the effects of the T locus in mice. Gene and haplotype frequencies in 32 unrelated patients suffering from spina bifida were studied. Five hundred and fourteen individuals from an isolated Newfoundland population have been typed and haplotyped for HLA and Factor B (Bf locus). The Bf gene frequencies were: S 0.704, F 0.226, F10.052 and S10.018. Tracing the haplotypes backwards on the various pedigrees it was shown that 202 HLA/Bf haplotypes had been introduced into the study population. Some HLA alleles always appeared in association with particular Bf alleles andvice versa.Bf S1had entered the population four times, always with HLA—A9, B12. Bf F1had entered three times, always with HLA—B18. There were 17 entries of HLA—Al, B8 that had given rise to the 78 such haplotypes that were now present in the population; all were Bf S. There were 68 family units informative for Bf. They accounted for 240 children and 304 informative meioses.
