SummaryThis study reports on five adult patients with severe atopic dermatitis who were treated in our department with a three month course of cyclosporin*All patients demonstrated significant clinical improvement with minimal side effects. We conclude that provided safety guidelines are observed, cyclosporin can be used with good effect in those patients where conventional treatment is unsatisfactory. This paper also reviews the use of cyclosporin in atopic dermatitis and includes a brief discussion of how it may alter pathophysiologic mechanisms.

SummarySkin changes associated with long term hydroxyurea therapy have only rarely been reported and are not widely known. This report concerns a patient in whom extensive and debilitating skin changes developed secondary to hydroxyurea therapy. In addition to previously reported manifestations such as ichthyosis, a dermatomyositis‐like eruption on the dorsal hands, facial erythema and accelerated development of premalignant and malignant skin lesions, the patient developed an erosive eruption on the glans penis. He also developed a rapidly enlarging viral wart which clinically resembled a squamous cell carcinoma and histologically showed marked cytological atypia. It is possible that hydroxyurea caused accelerated photoageing and also contributed to the rapid growth of the viral wart and the atypical histological features. The time course of the clinical changes suggests that they result from direct toxicity of hydroxyurea on the basal layer of the epidermis and mucosal surface

SummarySkin biopsies from patients with psoriasis and normal controls were examined for the expression of cell adhesion molecules including intercellular adhesion molecule‐1 (ICAM‐1) endothelial leukocyte adhesion molecule‐1 (ELAM‐1), HECA‐452 and 4D10, using an immunoperoxidase techique. This study demonstrates that psoriatic skin exhibits a wide variety of markers of endothelial cell activation which are either induced or increased expression (ICAM‐1, ELAM‐1&4D10). Moreover, ICAM‐1&HECA‐452 are also on leukocytes. These antigens may facilitate the adhesion of inflammatory cells to endothelium and antigen‐presenting cells in psoriatic skin. Thus, they may play a role in faciliating the infiltration of leukocytes

SummaryInterferon alfa‐2b is an effective but expensive way of treating basal cell carcinoma. In this paper, the cost‐effectiveness of interferon relative to conventional treatment is considered. Circumstances where surgery is contra‐indicated and would not otherwise be performed are not considered.The cost of achieving cure by interferon alfa‐2b in the form of Intron A® is compared with the cost of conventional treatment in four situations; simple lesions, lesions in which protacted healing is expected, lesions in areas where cosmetic result is important and large lesions where hospitalisation for skin grafting may be necessary.For simple lesions, treatment with interferon alfa‐2b costs between $500 and $760 more per cure than conventional treatment. However, it is less expensive than conventional methods in cases that would require extensive home nursing or hospitalisation. The difference in cost in simple cases also represents the price which must be paid for the better cosmetic result achieved by interferon alfa‐2b.In conclusion, interferon alfa‐2b adds another effective option to the treatment of BCC but it is expensive. SummaryNickel contact dermatitis appears to be almost exclusively a disease of females despite the increasing exposure of males to nickel. Successful murine models of nickel allergic contact dermatitis have been described. The purpose of this study is to investigate the kinetics and specificity of the response in this model and to examine if any differences exist between male and female. Mice were sensitised epicutaneously with nickel sulphate in aqueous solution of varying concentration, volume and duration of application. Following intradermal challenge, dose dependent response kinetics which approximated linearity were demonstrated upto the point of toxicity. Sensitised mice were challenged with Cobaltous chloride, Chromic chloride and Cupric sulphate and demonstrated no evidence of cross sensitivity to cobalt or chrome. Copper produced an irritant response making interpretation difficult. Earlier and stronger responses were observed in female mice, however these differences fell short of statistical significance. The results of the present study therefore establishes a reliable model for nickel hypersensitivity, that demonstrates both specificity and dose dependent kinetics without significant sex differences.
