SUMMARYOver a period of eight years 256 cases of suspected candidacies were investigated in private dermatological office practice by direct microscopic and cultural methods. Laboratory investigation cannot define the limits of disease with precision and the significance of the laboratory findings can only be made by the clinician, who cannot delegate responsibility for the final diagnosis. The majority of cases of intertriginous candidacies appeared to be secondary to intertriginous (seborrhoeic) dermatitis, diabetes, pregnancy or neoplasia, and primary candidiasis without a predisposing factor was not common. Gandidal balanitis was the clinical state most frequently associated with previously undiagnosed diabetes. Because of the inadequate response of intertriginous candidiasis to topical polyene antibiotics and the frequency of a co‐existing dermatitis, it was felt that hydroxyquinoline (e.g. vioform)—steroid preparations are the best available method of treatm

SUMMARYThe light sensitivity of porphyries catenae predisposes to an early ageing of the skin. In Australia where there is a high incidence of early ageing of the exposed skin, porphyries might easily remain undiagnosed.The classical manifestation of porphyries catenae tar da, the traumatic blistering, is an intermittent phenomenon and appears only over exposed skin areas showing the clinical manifestations of actinic elastics. The changes of the fine elastic fibers of the dermal papillae and the damaged basement membrane of the demo‐epidermal junction predisposes the aged skin to derma‐epidermal blister formation. The ultimate cause of the blistering is not photo‐ sensitivity, but probably a toxic factor which further damages the derma‐epidermal connections without the intervention of inflammatory phenomena.Porphyries will cause blistering in aged skin, but various non‐specific, non‐diagnostic manifestations, due to photo‐sensitivity occur in skin areas without elas

SUMMARYMalignant melanoblastomas (commonly referred to as melanomas or melanoblastomas) may arise from clinically normal looking skin or in other lesions, the commonest of which is the soft pigmented nevus (Melanocytoma or mole). In the latter case they commonly arise from junction navy or the functional component of a compound naevus, but very rarely from purely dermal naevi. The lesions with which they are more likely to be confused clinically are listed but all lesions concerned in the differential diagnosis of a squalors carcinoma may have to be considered, particularly for amelanotic lesions.Diagnosis is either made or confirmed by the microscope, and even here, certain difficulties (which are mentioned) may be encountered. SUMMARYThis would appear to be the first case of swimming pool granuloma recognized and reported in Australia. An organism identified as mycobacterium balnei was isolated from the lesion. Wide excision and split skin graft has given a satisfactory result and at present no sign of relapse of the disease can be found. Adequately wide excision and the technique of surface occlusion of the infected skin lesion prior to surgery may have helped to prevent relapse of the lesion. A new type of local therapy, which however has not been applied in this case, namely the application of heat therapy to the diseased skin, has been suggested for future cases which do not show spontaneous remission within a reasonable period, and the rationale of this new treatment has been discussed.Although most overseas reports have been of epidemics caused by gross infection of swimming pools, such sporadic cases, as the one reported here, must occur. The awareness of such an entity will facilitate the recognition of further cases.
