SummaryMelanonychia striata refers to a pigmented (tan‐brown‐black) streak due to the presence of melanin in the nail plate and/or nail bed. The streak is caused by a focal increase of the number and/or function of normal appearing or of abnormal melanocytes. Pigmented bands, often involving multiple digits, are prevalent in individuals of the more deeply pigmented races and, in them, are considered to be “normal” in the vast majority of instances.Among the causes of melanonychia striata are neoplasias including naevocytic naevi, atypical melanocytic hyperplasia, and acral lentiginous melanoma. A series of cases is presented to illustrate these oncological lesions. Until further knowledge is gained, surgical removal is recommended for those lesions resulting in melanonychia striata which fall into the neoplastic group.It is concluded that much more study is required to determine the biological significance, natural history, and outcome of these periungual melanocytic neo

SummaryA case of acrodermatitis enteropathica is reported in which there was a clinical response to zinc therapy despite a normal pretreatment plasma zinc level. Unusual features are the absence of diarrhoea and the large doses of zinc required for control of the disorder.

SummaryEleven patients with moderate to severe Darier's disease were treated with an oral aromatic retinoid, R010 9359 in a dose of 1 mg/kg/day for six weeks. Three patients were clear after six weeks, seven patients showed marked improvement and one patient showed moderate improvement. Pruritus, palmo‐plantar desquamation and hair loss proved troublesome in some patients but this was reversed by reduction of the dose or cessation of treatment. One patient showed moderate elevation of liver transaminases, thought to be significant. SummaryEight patients who developed unusual xanthomatous plaques with a tendency to ulceration and scarring and subcutaneous extension are described. Six patients had an IgG monoclonal serum paraprotein. Two patients had multiple myeloma and one patient had a plasmacytic lymphoma. Neutropenia, hypocomplementaemia and cryoglobulinaemia were variable features. Sixteen skin biopsies showed a consistent pattern of xanthogranulomatous nodules separated by zones of necrobiosis and an admixture of prominent giant cells. Lymphoid or plasma cell nodules may occur in the biopsies. Necrobiotic xanthogranuloma belongs to a range of xanthomas which has been associated with dysproteinaemia and lymphoreticular malignancy.

SummaryOne hundred and forty‐nine soil samples were collected from 27 suburbs within the Illawarra areas of New South Wales.Microsporum gypseumwas isolated from 74.5%,Microsporum cookeifrom 56.4% andKeratinomyces ajelloifrom 55.7% of soil samples. Two or more of these dermatophytes were simultaneously isolated from one soil sample on 93 occasions (62.4%).Trichophyton terrestrewas not isolated, howeverTrichophyton mentagrophyteswas isolated on two occasions, although repeat cultures of soil from the vicinity proved negative. SummaryTwenty patients with mycosis fungoides were treated with photochemotherapy using oral psoralens and long wave ultraviolet light (PUVA) over a two‐year period. PUVA was effective in producing a diminution of cutaneous deposits of mycosis fungoides with each clinical pattern of presentation. SummaryTwo patients with primary disseminated mucocutaneous histoplasmosis are reported. As there was no focus of infection, active or healed, in the lung or other internal organs, it is assumed that a primary cutaneous site was the source for dissemination to the skin and mucous membrane.
