摘要:
SUMMARY1. A group of seven different liver function tests was performed in ten cases of generalized exfoliative dermatitis of three weeks' to seven years' duration, and in ten controls suffering from various other skin ailments. In most of the cases the tests were repeated at intervals. In some of the test methods (cephalin flocculation, thymol turbidity, alkaline phosphatase, Schlesinger's urine test, serum globulin and albumin/globulin ratio), abnormal readings were found three times as often in cases of the disease as in the controls, but this was not found to be of statistical significance. Two other test methods showed a considerable lowering of the serum albumin level and of the hippuric acid excretion in the ten cases reviewed, as compared with the controls. These findings are of statistical significance.2. Eesults of the series of liver function tests revealed that in cases of generalized exfoliative dermatitis the serum albumin levels and the hippuric acid excretion readings rose, as the exfoliation and the general state of the patient improved, and fell, as the patient's skin condition deteriorated.3. Experimentally, by giving or withholding AOTH or cortisone, exfoliation could be stopped or started at will, and it was found that the serum albumin and hippuric acid excretion tests would follow the previously described pattern.4. Eesults in the first group of test methods likewise varied with the deterioration and improvement of the skin condition, as estimated by the amount of exfoliation, but were not reliable.5. Post‐mortem examination of the liver in one patient with generalized exfoliative dermatitis showed liver damage. Liver biopsy and post‐mortem examination in a further case showed abnormal histological changes in the liver. In both these cases, only the serum albumin levels and the hippuric acid excretion tests showed gross abnormal readings. There is suggestive evidence that the hippuric acid excretion test and the serum albumin level may be particularly affected in this condition.6. Four patients in this series had enlarged livers at some time during their exfoliative phase (two of them prior to exfoliation), compared with one patient of the controls.7. An impression was gained that generalized exfoliative dermatitis, which lasts more than three weeks, may produce a certain degree of liver damage, which is reversible after the skin condition has cleared and evidence is brought forward to support this hypothesis.8. It is suggested that ACTH or cortisone may be of use in an attempt to determine in other disease processes, as in this series, which organ is primarily diseased in cases where various organs are affec

摘要:
SUMMARYA case of chromoblastomycosis, a superficial mycotic disease, is reported as having occurred in the east coast area of the North Island of New Zealand.Chromoblastomycosis affects the male more frequently than the female, usually in the age group thirty to fifty years. This case is that of a male aged seventy years, and as is usual, an exposed part of the body–the arm only– was affected. The duration of the lesion was roughly three years. The clinical and histological appearances of the lesion were in conformity with those reported by Barrack in Queensland. The organism isolated on cultures was of the genusHormodendrum. The disease has never been reported in New Zealand previously to our knowledge. The fact that it occurred in an Oriental market gardener, domiciled for 47 years, makes it highly probable that the infection was contracted in New Zealand. It is our belief that this disease is present in New Zealand, and that it has not as yet been recognized, since the lesions may well resolve spontaneously or be destroyed by diathermy or surgery without histological examination or culture. There has been no evidence to show that the patient was engaged in work associated with heavily timbered country, although native bush is or has been within a radius of ten miles of the areas cultivated for market gardening in this case. The climatic conditions are essentially those of the temperate zones in latitude 35 to 40 degrees so

摘要:
SUMMARY1. Two hundred and twenty‐one plantar warts treated by X‐radiation at Royal Prince Alfred Hospital in 1946 were reviewed in 1948, and again in 1953.2. The cure rate under the age of twenty‐five years was 93%, and above that age, 52‐3%.3. Lesions (some of which may not have been warts) on pressure areas gave bad results.4. Ten of the 21 failures occurred in treating lesions that were at the time of review pure callosities.5. Two of these failures are briefly considered, and indicate the importance of correct diagnosis and the dangers of repetition of tr

摘要:
SUMMARY1. A method of using guinea‐pigs and mice to detect pathogenicity in strains ofC. albicansis described.2. Agglutination ofC. albicansby the slide method has been shown to be reliable.3. Seventy‐eight strains of yeast‐like organisms have been submitted to a number of tests to decide upon a method of identifyingC. albicans. The tests were fermentation reactions, formation of hyphae and chlamydospores in corn‐meal extract, animal pathogenicity tests, slide agglutination with an absorbed anti‐serum and colony appearance on eosin‐methylene blue agar. No single test was completely accurate.4. Eosin‐methylene blue agar is a valuable medium for isolating and detectingC. albicans.5. Slide agglutination gives the smallest number of errors where a reasonably rapid decision h
