This paper is intended to provide some guidance to people wishing to monitor quality in small scale community residential settings. It describes the results of a study in which 14 objective quality of life indicators were conducted in a sample of 14 community‐based staffed houses in order to assess the degree of overlap between measures within each of six quality of life domains: activity, autonomy and choice, housing quality, personal development, social and community integration, and social interactions. Thirty‐six people with a moderate to severe learning disability were interviewed concerning their accommodation service. The interview consisted of 49 items, grouped in five categories: material, physical, developmental, social and emotional well‐being. Three question techniques were used: open questions, closed questions and requests for opinions, augmented by line drawings depicting facial expressions (happy, sad, etc). The findings demonstrate that consumers can make an important contribution to the review of services they receive, but that the interview process is by no means easy. Implications for involving service users in the development of individual plans (IPs) are discussed. Based on the respondents' comments, suggestions are made for the development of accommodation services. The people with learning disabilities (mental handicap) originating from Westminster, England who were aged over 61 years in 1984 were compared with those in the same age group in 1994. Over the decade there had been a significant change in the nature of the residential provision; the proportion resident in hospitals fell and that in ‘community care’ increased. This study set out to describe and analyse the nature and consequences of care for a small number of informal carers (n= 9) providing full‐time care for people with learning disabilities discharged from long‐stay hospitals between 1987 and 1992. The findings showed that these carers, whilst not adversely affected by their caring roles, did experience restrictions on their personal and social lives as well as increased anxiety and physical fatigue. In contrast with findings from other studies of carers of people with learning disabilities, they had more difficulty with practical caring tasks/activities (e.g. providing help with social activities) than with problem behaviour. This small scale study took place in an integrated nursery class in an SLD school containing eleven children with severe learning difficulties (SLD) and six children without disabilties — referred to as ‘playgroup’ children. This study explored the attitudes of speech and language therapists (SLT) to intimate relationships among people with learning difficulties. A stratified random sample of SLTs (n=66) were sent a short, written scenario concerning two young adults with learning difficulties who were about to set up home together. Half the SLT's received a scenario in which ‘John’ was said to have aseverelearning difficulty, the other participants received a scenario in which ‘John’ had amildlearning difficulty. The two scenarios were otherwise identical. All participants were asked to rate the scenario using a semantic differential scale. Forty completed questionnaires were analysed and the results indicated relatively favourable attitudes toward someone with a mild learning difficulty. Attitudes toward a person with severe learning difficulties were significantly less favourable (p<0.001), however, it could not be concluded that the unfavourable attitudes were directly related to engaging in an intimate sexual relat

Strong experimental evidence exists for the use of differential reinforcement procedures in ameliorating SIB in people with learning disabilities. However, recently some researchers have pointed out the inadequacy of single element treatment approaches in dealing with all cases of SIB and have stressed the need for services to develop multi‐modal, integrative intervention programmes based on non‐aversive approaches. This case report describes a treatment package for severe self‐injurious behaviour containing elements of DRO, use of a Snoezelen Room, orientation cues and staff support. Results showed the treatment programme to have been highly successful, with a marked reduction in the subject's SIB, along with a general increase in positive behaviours. A six‐month follow‐up study demonstrated that this improvement had been maintained. Thus, it was concluded that this study offers support for the concept of a multi‐faceted treatment package and rather more tentatively for the use of a Snoezelen Room as a component of such a package for some
