AbstractThe tremorolytic effects of primidone and phenobarbital in essential tremor of hands and head were compared in a double‐blind, placebo‐controlled trial. Quantitative measurements of tremor were obtained in 15 patients by means of an accelerometric method. Only primidone proved to be superior to placebo in reducing hand tremor, suggesting that its tremorolytic effectiveness is largely dependent on the parent drug rather than its metabolite phenobarbital. AbstractA 6‐year‐old girl is described with a history of episodes of severe ataxia precipitated by fever since the age of 11 months and in whom mental and physical development was otherwise normal. The ataxia was associated with a complete vertical supranuclear ophthalmoplegia and normal horizontal eye movements. After resolution of the attack she had no abnormal movement of her limbs or clinical evidence of ophthalmoplega, though electro‐oculograms showed a presistent mild deficit of vertical saccades. In between the attacks hyperglycinuria, an elevated plasma glycine level (687 μmol L−1), and negative screening for organic aciduria suggest that she may have nonketotic hyperglycinaemia. AbstractThe occurrence of acute dystonic reactions (ADRs) due to domperidone administration in two young women is reported. In both patients, a typical polycystic ovary (PCO) syndrome was found. The possibility that the relative hyperestrogenism typical of PCO syndrome acted as a facilitating factor for ADR is discussed.

AbstractCarpal tunnel syndrome (CTS) is usually an idiopathic disorder. Certain occupations that require frequent flexion movements of the hand at the wrist are recognized as precipitating the development of CTS. Dystonia can cause similar excessive movements at the wrists. We report the clinical and electromyographic findings of two patients with idiopathic torsion dystonia (ITD), with frequent flexion postures of the wrists, in both of whom typical CTS developed in the more involved hand. It is concluded that nerve entrapment such as CTS has to be considered a possible complication in patients with ITD and other motor control disorders.
