Natural latex, a product of the tropical rubber tree, Hevea brasiliensis, has been shown to cause allergic reactions in an increasing number of individuals. Severity of reaction has ranged from mild contact dermatitis to anaphylaxis. Individuals with spina bifida, congenital urogenital anomalies, health care workers, and those employed in the rubber industry are at highest risk for immediate hypersensitivity to latex. Health care workers should be aware of this high risk to minimize exposure of patients.

This review touches on three areas of recent research on genetic lipid disorders. The first is the advent of a trial of gene therapy for homozygous familial hypercholesterolemia. Affected individuals are at risk for myocardial infarction by adolescence. The experimental therapeutic protocol is aimed at restoring function of the defective low density lipoprotein (LDL) receptor gene in patients by introducing complementary DNA encoding normal LDL receptor into a patient's hepatocytes ex vivo. The second area reviewed concerns an important but lesser known relative of LDL, lipoprotein (a). Concentrations of lipoprotein (a) vary a thousandfold among humans. High levels pose a strong, independent risk for early heart disease, but lipoprotein (a) determinations are not yet routine. Finally, an update is presented on the controversy over cholesterol screening and treatment in childhood.

The explosion in molecular technology has brought with it a new era in medical genetics. In the past, genes easiest to work with were those responsible for heritable disorders manifesting in childhood that were often the consequence of metabolic derangements. The disorders were detectable either at birth or soon after either because of physical appearance or the development of a clinical problem. It is now possible to identify genes that confer increased susceptibility to diseases that may not be manifest until late adulthood, such as cancer or heart disease. This review discusses conditions in which children may now be identified as genetically at increased risk for cancer development in childhood or adulthood, and current dilemmas regarding appropriate screening strategies for these at-risk individuals.

Knowledge about the molecular basis of several diseases is steadily increasing; however our knowledge regarding the molecular basis of cardiomyopathies is still limited. Although the etiology of many inherited metabolic diseases accompanied by cardiomyopathy has been identified in the past decade, many of these entities are extremely rare, so that genetic studies are difficult to perform. Despite this, in some entities, the molecular and genetic basis of cardiomyopathy was identified. However, understanding of the molecular basis of cardiomyopathy is still in the beginning stages.

For many years, the mainstay of pediatric genetic testing was chromosomal analysis. Based on a technology developed in the 1950s and refined in the 1960s, this approach permitted the detection of abnormalities of chromosome number and some structural rearrangements. The past decade has seen an explosion in knowledge about the genetic basis of disease, and accompanying this, the development of new types of diagnostic tests. This article reviews some of these new approaches, and provides a practical guide for the pediatrician who is increasingly called upon to arrange for such testing and to interpret the results.

This review of patient and parent education, office laboratory procedures, and urinary tract infections is written by three pediatricians in a group practice in Pittsburgh, Pennsylvania. The patient and parent education section covers the goals of child rearing and history taking (with emphasis on the “vulnerable child”), and reviews discipline, sleep problems, fever, smoking, and injury prevention. The office laboratory procedures section reviews the recent Clinical Laboratory Improvement Act of 1988. The urinary tract infections section reviews recent advances in diagnosis, treatments, prevention (the issue of circumcision), and radiologic studies.
