The objective of this study was to determine the effects of nitroglycerin on ventricular dimensions. At the time of corrective cardiac operations, silver-tantalum markers were sutured to one or both ventricles of 11 patients without clinical evidence of coronary artery disease. Following recovery, cineradiograms were exposed at 30 frames per second and distances between markers were measured before and after 0.6 mg. of nitroglycerin. In all patients, end-diastolic and end-systolic dimensions decreased within 2 to 6 minutes. Right ventricular end-diastolic and end-systolic lengths decreased by an average of 5.0 per cent and 3.6 per cent of control values, respectively, while left ventricular end-diastolic and end-systolic dimensions declined by an average of 6.2 per cent and 5.9 per cent of control, respectively. The diminutions of end-diastolic ventricular dimensions approximated 30 per cent of the stroke volume. Systolic excursions decreased by an average of 13.3 per cent of control for the right ventricle, while left ventricular excursions decreased in four of the six patients. Systolic, mean, and diastolic arterial pressures decreased in each patient after nitroglycerin. Cardiac output fell from an average of 5.6 to 5.0 L./min., decreasing in each of the six patients in whom it was determined, while stroke volume fell from an average value of 65 ml./beat during the control period to 55 ml./beat after nitroglycerin.Thus, nitroglycerin reduces both arterial pressure and ventricular dimensions and in view of the relationship between ventricular size, myocardial tension, and oxygen consumption, this action tends to reduce cardiac oxygen requirements and may explain, at least in part, the effectiveness of the drug in angina pectoris.

The short circulatory pathways of infants cause rapid systemic and pulmonary circulation times. So rapid are they that the usual methods for calculating left-to-right shunts frequently yield a considerable overestimation. A new formula was described for use with injections into the left heart and arterial sampling. This formula applied to 11 infants and 7 older patients showed a significantly better correlation with oximetric calculations than did the right heart injections. This method also will reveal the presence of a left-to-right shunt that is of insufficient magnitude to distort dye curves obtained following right heart injections.

The &bgr;-adrenergic blocking drug pronethalol was effective in the treatment of a patient with exercise-induced paroxysmal ventricular tachycardia. Adrenergic stimuli are considered important in initiating this arrhythmia. The mode of action of pronethalol is briefly discussed in relation to other antiarrhythmic agents.

This paper reports six cases (age range 8 months to 69 years) of iatrogenic foreign bodies lodged within the heart or great vessels. Four cases were produced during left heart catheterization by breakage of a metal stylet, and two after the insertion of a Pudenz valve (for hydrocephalus) which embolized into the right ventricle and pulmonary artery.In five of these patients, the foreign body was intracardiac (three in the left ventricle and two in the right ventricle and pulmonary artery) and the open-heart surgery was utilized for successful removal. In the sixth patient, the stylet was removed from the innominate artery via a closed procedure. Five of the six patients survived (including the youngest and oldest) and their postoperative course was uneventful. The one death followed removal of the right-left atrial foreign body and total aortic valve replacement for calcific stenosis, and failure appeared related to the basic disease rather than to the foreign body.Such complications have rarely been reported in the literature. The high ratio of success, if treatment is prompt before the formation of clots (which is rapid), needs full appreciation, and emphasis is given to the benefits of prompt surgery for removal.

The relative importance of arterial oxygen tension and hydrogen ion concentration in regulating pulmonary arterial pressure was evaluated in a 43-year-old woman with hypoventilation and obesity. It was shown that hydrogen ion concentration played a dominant role in the regulation of her pulmonary circulation.

The origin of an audible immediate diastolic murmur in 10 of 22 patients withatrioventricular defects was investigated by intracardiac phonocardiography. It was shown to arise at the site of the atrial septal defect, and all other possible causes of such a murmur were excluded.The production of the murmur depended on the combination of mitral regurgitation and an atrial septal defect of such a size as to permit an interatrial pressure gradient in early diastole.

Origin of both great vessels from the right ventricle in association with a ventricular septal defect was found in seven of nine autopsied patients with the clinical features of the trisomy-18 syndrome. In four of these patients trisomy-18 was confirmed by chromosomal analyses. This experience indicates that origin of both great vessels from the right ventricle may in fact be an additional component of the trisomy-18 syndrome. A complication of right heart catheterization is described in which the tip of the catheter became impacted in a vein connecting to the coronary sinus, requiring thoracotomy and removal under direct vision. This occurred in a 2-year-old patient with an anomalous inferior vena cava with azygos continuation, and it is suggested that this anomalous course may well have contributed to the development of this complication.
