Six hundred and seventy-six healthy subjects between 5 and 77 years of age have been studied by radioelectrocardiography. These subjects were studied before exercise, during a Master's two-step test, and after exercise. Orthostatic ST-T changes occurred in 32 of 200 subjects (16 per cent). The results indicate that the positioning of the bipolar chest leads is crucial in eliminating “abnormal” ST-T changes caused by changes in posture and position. Criteria for orthostatic ST-T changes have been presented. It is concluded that more rigid criteria for “definitely abnormal” ST-T changes during radioelectrocardiography should be developed, so that coronary artery disease will not be overdiagnosed, nor will significant ST-T changes be regarded as “normal variants” occurring with activity.

A case is presented of a 16-month-old white boy with a communication or tunnel between the right sinus of Valsalva and the outflow tract of the left ventricle. The presence of a murmur at birth and the subsequent clinical course suggest that the tunnel was congenital. The physical, electrocardiographic, phonocardiographic, and aortographic findings were quite similar to those of “aortico-left ventricular tunnel” as described by Levy et al.1The tunnel was occluded surgically, but failure to appreciate the presence of severe aortic stenosis contributed to a fatal outcome.Postmortem study was carried out, and the relationship of the findings of aortico-left ventricular tunnel to aneurysm of the sinuses of Valsalva and the Marfan syndrome is briefly discussed.

Chronic atrial flutter was found in two teenage brothers with classical features of the Marfan syndrome. Since neither patient has a gross cardiovascular defect, it is postulated that the arrhythmias may be secondary to an alteration in the fine structure of the atrial myocardium.

A case of a mycotic false aneurysm of the ascending aorta and aortic sinus caused by coagulase-negative staphylococcal endocarditis following the placement of a Starr-Edwards prosthesis for rheumatic aortic stenosis is reported. The formation of the aneurysm caused aortic insufficiency, and terminally the prosthesis was torn from the aorta. The occurrence of mycotic aneurysms in the aorta and their etiology and pathogenesis are discussed. Cases of postoperative false-aneurysm formation of the ascending aorta are reviewed.

A case of partial anomalous pulmonary venous drainage with intact interatrial septum associated with mitral stenosis is reported. The roentgenogram showed plethora confined to the anomalously draining lung tissue, i.e., right upper amd middle lobes, and when the patient developed pulmonary edema, rales were confined to normally draining lung tissue, i.e., left lung and right lower lobe.The pulmonary capillary “wedge” pressures recorded from the normally draining lung tissue reflected the left atrial hypertension, while “wedge” pressures recorded from anomalously draining lung tissue were much lower and reflected the right atrial pressure. Left heart catheterization confirmed the presence of mitral stenosis. Autopsy examination revealed congenital mitral and aortic stenosis, anomalous venous drainage from the right upper and middle lobes and an intact interatrial septum. The differential pulmonary plethora, the differential pulmonary edema, and the differential pulmonary capillary wedge pressures suggested the correct diagnosis during life.

Lesions causing obstruction to the outflow of blood from the left ventricle may reside in the ascending aorta, the aortic valve, or the outflow tract of the left ventricle.Conditions involving the ascending aorta are usually congenital, and designated as supravalvular aortic stenosis. A tendency is present for this condition to be associated with mental retardation, stenosis of peripheral pulmonary arteries, stenosis of branches of the aortic arch, and a peculiar facies. Secondary narrowing of coronary arteries may occur.Obstruction at the aortic valve is usually represented by a stenotic lesion. In some instances, the basis for stenosis is congenital; in others, it is acquired either through the direct effects of rheumatic endocarditis or through calcification of an acquired or congenital bicuspid aortic valve.Atresia of the aortic valve causes death in infancy.Subaortic stenosis is a complicated subject. It may be represented by a primary anomaly of the left ventricular outflow tract or may be secondary either to conditions primary in the mitral valve or to generalized diseases of the myocardium.In some instances of subaortic stenosis, this process is one part of a complex cardiac malformation.

Emphasis is placed upon the correlations between clinical, hemodynamic, and angiographic features that permit an accurate diagnosis. An exact definition of the problem in each patient has become of great importance because of rapid advances in cardiac surgery, which now offers specific treatment for each of the syndromes described.

摘要:
Currently available diagnostic methods and surgical technics now allow accurate assessment and effective correction of the majority of lesions producing obstruction of the left ventricular outflow tract. On the basis of anatomic location these lesions can be divided into supravalvular, valvular, and subvalvular stenosis, and valvular lesions further can be subdivided into congenital and acquired categories. Temporary cardiopulmonary bypass with a pump oxygenator provides sustained support of the circulation for operation, while surgical technics employed are selected on the basis of etiology and location of the lesion. Experience gained in operations on 296 patients with obstructive lesions of the left ventricular outflow tract since 1956 now has demonstrated that these operations usually can be performed with acceptable risk and with excellent functional results.
