A case of perforation of the left ventricle during selective cineangiocardiography in a 27-month-old child with aortic stenosis is reported. Perforation was visualized during the power injection, and subsequent cardiac tamponade developed. Immediate pericardiocentesis resulted in rapid improvement and the child made an uneventful recovery. At cardiac surgery for aortic valvotomy 5 days later no abnormalities of the pericardium were noted but there was a small area of discoloration on the posterior left ventricular free wall. Prevention of poor catheter positioning is suggested as the major preventive measure to avoid such complications during selective ventricular angiocardiography.

The case of a 10--year-old boy is reported because of the association of the Ehlers-Danlos syndrome with bifidity of the pulmonary artery and anomaly of the aortic arch.The cardiovascular aspects of this connective-tissue disorder are reviewed, and the possibility of another common feature with the Marfan syndrome is briefly discussed.

Standardization of an exercise electrocardiographic test for myocardial ischemia is desirable. Standardization of exercise should be based upon a standardized challenge to the coronary circulation, not upon a standardized challenge to the skeletal muscles, for the stresses upon the heart are not identical.With the use of heart rate during exerciseas an index of cardiac stress, the levels of exercise prescribed on the basis of age, sex, and weight in standard “two-step” test tables produce widely varying levels of cardiac stress-sometimes too low to induce significant ST-segment depression in patients with angina pectoris and, occasionally, much greater than necessary to produce such diagnostic electrocardiographic changes.The graded exercise test, using 85 per cent of the maximal age-predicted heart rate that may be induced by exercise, produces a closer approximation of a standardized cardiac stress, regardless of age, sex, or body build. Its use with angina pectoris patients and normal subjects results in increased sensitivity (fewer false negatives) and increased specificity (fewer false positives).Use of the graded exercise test as described is considered not to increase the hazard of testing, and perhaps even to decrease it.

Indicators can be used to measure myocardial blood flow if the myocardial circulation can be isolated from other segments of the systemic circulation.Methods of measuring myocardial blood flow can be divided into groups according to the technic employed to provide “isolation” of the myocardium.There is no practical method of proved accuracy whereby coronary blood flow can be measured in man in absolute terms of ml./min.All the so-called clearance methods measure myocardial blood flow in flow/volume units or ml./min./100 Gm.Myocardial blood flow measurements in these flow/volume units have not proved useful in separation of normal individuals from those with ischemic heart disease at rest, but have been useful in the study of acute intervention such as exercise or the administration of drugs.

An objective method for the quantitative measurement of the coronary blood flow in ml./min. using a coincidence counting system with rubidium-84, a positron emitter, is described. Accidental coincidence counts arising from independent annihilation of positrons were greatly reduced by simple shielding.The coronary blood flow was calculated by the extrapolation of the clearance values to zero time. At this time the myocardial extraction ratio of rubidium-84 is unity and clearance is equal to flow. The accuracy of thismethod was demonstrated on the isolated perfused dog heart.It was found that the rate of change of the extraction ratio of rubidium-84 in the isolated perfused dog heart and in man was not altered with changes in flow.The finding that nitroglycerin increases the coronary blood flow in patients without coronary artery disease while decreasing it in patients with coronary artery disease was confirmed.

Patients with myocardial infarctions may show a paradoxic precordial outward systolic movement that can be recorded by the kineto-cardiographic technic and often is felt at the bedside.Three types of changes occur. The first is an early systolic outward movement, which is abnormally long in duration but not sustained throughout systole. The second is a sustained outward movement during systole called a “bulge,” and the third is a late systolic outward movement.Bulges can be recorded in approximately two thirds of the patients with known infarctions with the largest outward movement most frequently noted in or near the V3area.A few patients with angina may have a bulge at rest; however, apparently about 30 per cent develop a bulge on exercise.
