The clinical, hemodynamic, angiographic and, where available, surgical and postmortem findings of 10 cases of muscular subaortic stenosis have been reviewed and, from the evidence at hand, it would appear that the majority of these cases had as the basis for the stenosis, idiopathic ventricular septal hypertrophy (asymmetrical hypertrophy of the heart).3This septal muscle mass leads to obstruction of the right as well as the left ventricular outflow tract, in addition to impeding diastolic ventricular filling as a result of low ventricular compliance. These features give rise to a unique clinical, hemodynamic, and angiographic picture.

Two cases of coronary artery heart disease are presented in which the Master two-step exercise test provoked short bursts of paroxysmal tachycardia. In one patient, nodal tachycardia was present; in the other, the tachycardia was either ventricular or nodal with aberrant conduction. Difficulties in assessing the probable etiology of paroxysmal tachycardia after exercise, which appears as an isolated finding in the coronary age group, are discussed.

For the present, the primary obligations of this communication are to acknowledge that the electrocardiographic patterns of myocardial ischemia, injury, and necrosis may appear in asymptomatic, young white males who have low-intensity systolic murmurs; to record that any implication that these electrical deviations result from obstructive coronary disease cannot be substantiated by coronary arteriography; and to suggest that individuals possessing these diagnostic features may represent a distant, recognizable clinical entity the etiology of which remains obscure.

Nineteen observations were made in 12 patients with early essential hypertension. The comparative effects upon mean arterial blood pressure, cardiac index, and total peripheral resistance of the acute and long-term administration of chlorothiazide were studied.With acute administration of the drug, a 10 per cent drop in mean arterial blood pressure, a decrease of 18 per cent in cardiac index, and an increase of 7 per cent in peripheral resistance were observed.With long-term administration, a decrease of 21 per cent in mean arterial blood pressure, an increase of 4 per cent in cardiac index, and a drop of 26 per cent in peripheral resistance were found.The conclusion is drawn that reduction of mean arterial blood pressure with acute administration of chlorothiazide is due to decrease in cardiac output, whereas with long-term use it is due to reduction in total peripheral resistance.

A comparative study was made of the acute effect of chlorothiazide upon renal hemodynamics in eight normotensive subjects and 10 patients with early essential hypertension.It was observed that blood pressure remained unchanged in normotensive subjects but was significantly reduced in hypertensive patients. Glomerular filtration rate was decreased similarly in both groups, whereas renal plasma flow was not modified in either. Osmotic clearance increased in all subjects, free water clearance diminished in normotensive (overhydrated) subjects, whereas in the hypertensive (not overhydrated) patients free water reabsorption increased.It is concluded that chlorothiazide reduces blood pressure in hypertensive patients but not in normotensive persons; that the kidney reacts in the same fashion in normotensive and hypertensive subjects under the action of the drug; and that the alterations in renal function observed with its use in hypertensive subjects are probably unrelated to changes in blood pressure.
