The percentage changes in the cardiac output, stroke volume, and heart rate were determined in seven healthy habitual smokers after smoking, after glucose, and after smoking preceded by glucose. No significant changes occur after glucose. There is a significant increase in cardiac output, stroke volume, and heart rate after smoking. Pretreatment with glucose did not change the heart-rate response to smoking. Nevertheless, the cardiac-output response was diminished. This decrease is due to a block in the increase in stroke volume induced by smoking.

The coincidence counting technic was used to measure the clearance equivalent of rubidium-84 as a function of coronary flow by the human heart in vivo. This technic makes possible a distinction of radioactivity of the heart muscle from the surrounding tissue, eliminates lead collimation, and is more sensitive than the singles technic.In experiments on isolated dog heart it was found that changes in rate of perfusion of the coronary arteries failed to influence the myocardial extraction ratio of rubidium-84. Alterations in heart rate were also without effect.Nitroglycerin increased the clearance equivalent of rubidium-84 in patients without coronary heart disease. In patients with coronary artery disease the drug led to fall in clearance equivalent.The method has been helpful in distinguishing between normal individuals and those with coronary artery disease.

To evaluate the hemodynamics of large and small vessels in patients with complete arterial occlusion and patients with vasospasm, digital flow, digital pulse contour, small-vessel resistance, flow-to-pulse ratio, large-vessel pressure drop, pulse velocity, and distensibility were compared to values found in normal subjects.[see figure in the PDF file]In the normal subject following orally administered alcohol, small-vessel resistance decreased but large-artery pressure drop, pulse velocity, and distensibility of the capacity vessels were not altered significantly.In arterial occlusive disease, small-vessel resistance was essentially normal in the absence of small-vessel occlusion; a high large-artery pressure drop, rounded pulse contour, and decreased pulse amplitude were observed. These are interpreted as being due to the damping effect of the narrowed, elongated collateral flow pathway that develops around the site of the occlusion. Pulse velocity also was decreased whereas flow-to-pulse ratio was increased in patients with occlusive disease. Large-vessel hemodynamics were unchanged after alcohol, suggesting lack of effect of the dilator on the collateral vessels.In vasospasm there was evidence of increased large-artery and small-vessel resistance and decreased distensibility of the capacity vessels, all of which returned toward but not completely to normal following alcohol.Digital pulse amplitude was found not to correlate well with flow, particularly in patients with arterial occlusive disease.

Thirty-four patients with ventricular septal defect and aortic regurgitation, representing less than 5 per cent of our patients with ventricular septal defect, are discussed.A loud, systolic murmur, characteristic of ventricular septal defect, is noted during infancy, whereas evidences of aortic regurgitation (protodiastolic murmur and wide pulse pressure) does not usually appear until sometime between 2 and 10 years of age.Clinical and catheterization data indicate that the principal hemodynamic load is aorticregurgitation, whereas the ventricular septal defect does not usually result in a large pulmonary blood flow or high pulmonary arterial pressure. In about 50 per cent of the patients, a significant pressure gradient across the right ventricular outflow tract exists.Detailed pathologic studies indicate that the ventricular septal defects are high and anterior and encroach to a greater or lesser degree on the membranous bulbar septum. The right coronary cusp is the one most severely involved, and, by its prolapse, causes aortic regurgitation; the noncoronary cusp is always less severely affected. The anatomic basis of the pressure gradient observed across the right ventricular outflow tract is not always clear.

A group of 12 subjects exhibiting a behavior pattern A, associated with a high incidence of clinical coronary artery disease, and a second group of subjects exhibiting a converse type of behavior pattern B, were fed a standard fat meal, before and after which studies were made of serum lipid and cholesterol changes. In addition, studies were made of their bulbar conjunctival vasculature.It was found that the average fasting serum triglyceride, cholesterol, and phospholipid values of the “A” subjects were significantly greater than those of the “B” subjects. Following the meal, the average serum triglyceride rose much higher and remained relatively much higher and longer in subjects with pattern A than in those with pattern B. The serum free fatty acid content was essentially the same in both groups, both before and after the meal. The observed differences in serum triglyceride behavior did not appear to be due to differences in the circulating phase of the heparininduced lipoprotein-lipase system.Following the meal, marked sludging and capillary ischemia were found in the conjunctival tissue of 10 or 12 subjects with pattern A and in only three of the 12 subjects with pattern B.

Left ventricular volumes were determined in 86 patients with aortic and mitral valve disease by use of a biplane angiocardiographic technicin conjunction with cardiac catheterization. Total stroke output of the left ventricle was grossly increased by severe degrees of valvular incompetence. Although both end-systolic and total stroke volume varied directly with the end-diastolic volume, there was no correlation between end-diastolic pressure and either total stroke volume or end-diastolic volume.
