The relationship between the electrocardiogram and the weights of the partitioned cardiac chambers of 185 adult hearts was analyzed. Several electrocardiographic variables were correlated with the combined weight of the left ventricle plus interventricular septum (LV + IVS), the weight of the right ventricle[see table in the PDF file](RV), their ratio (LV+IVS/RV), and the weight of the atria.The voltage of the R wave in aVLwas the best criterion for the diagnosis of left ventricular hypertrophy (LVH): among the cases in which the combined weight of LV+IVS and the ratio of LV+IVS/RV were elevated, 50 per cent had an R exceeding 7.5 mm., compared to a frequency of 4 per cent among the cases in which these two anatomic measurements were normal.The voltages of SV1, SV2, RV5, RV6, the sum of SV1plus RV5or V6, the sum of R plus S deflections in the precordial lead displaying the largest QRS complex, the S in Veand the Lewis index were larger in LVH than in normotrophic cases, but their diagnostic score was slightly inferior to the voltage of the R wave in aVL.Among all the cases with heavy LV+IVS, the diagnosis of left ventricular hypertrophy was most frequent with severe left ventricular[see table in the PDF file]hypertrophy, less frequent with moderate left ventricular hypertrophy and least frequent with combined hypertrophy.Left axis deviation was rare in cases with pure right ventricular hypertrophy. The frequency of left axis deviation in normotrophic hearts and in cases with heavy LV + IVS was about the same (52 per cent and 46 per cent, respectively); it was less common (33 per cent) among the cases in which the only anatomic evidence of left ventricular hypertrophy was a high LV + IVS/RV ratio.An axis greater than +60° was common in cases with pure RVH and not rare in normotrophic hearts (67 per cent and 15 per cent, respectively). An axis greater than +60° was not found in cases with heavy LV + IVS unless infarction was present as well. A QRS axis greater than +90° was almost pathognomonic of right ventricular enlargement. It was, however, of limited diagnostic value, being present in only a small minority of the cases with right ventricular hypertrophy.The timing of the intrinsicoid deflection was of limited value for the diagnosis of LVH and was not helpful for the diagnosis of RVH.The position of the transitional zone did not correlate with either RVH or LVH.The QRS duration tended to be longer in cases with LVH. This prolongation, however, seemed to be related not so much to the hypertrophy per se, but rather to the infarcts so frequently present in these large hearts.A very poor correlation was found between the right-precordial-lead patterns and RVH: as a whole the performance of the electrocardiogram in the diagnosis of RVH was highly unsatisfactory.Increase in weight of the left atrium tended to increase the P-wave duration and to shift the P axis to the left without affecting the voltage.No obvious relationship could be demonstrated between the weight of the right atrium and the voltage, duration, or axis of the P waves.

Pargyline exerts its antihypertensive effect via a predominant reduction in peripheral vascular resistance, whereas cardiac output is not altered significantly. The effect on renal blood flow is inconsistent, but a significant reduction in glomerular filtration rate does occur. Therefore the drug should be used with appropriate caution in patients with significant impairment of renal function. The hemodynamic response observed with pargyline is similar to that obtained with other MAO inhibitors.

A group of 6,400 volunteers aged 15 to 64 employed in the pharmaceutical industry in Basle, Switzerland, were examined for occlusions of limb arteries. Each subject was given a screening examination including a cardiovascular history, palpation, and electronic recording of foot and wrist pulses. An additional, more detailed examination was done on 294 subjects with pathologic or doubtful screening findings. This included arteriography for 83 subjects.Occlusions of peripheral arteries were found in 99 extremities of 75 subjects. One sixth was located in arteries of the upper extremities; 14 per cent concerned the iliac, 49 per cent the femoral arteries, and 23 per cent the arteries of the lower leg. In the age group 15 to 39 years occlusions occurred as frequently in women as in men, while a male predominance of 1.6:1 was noted in the 40 to 64 age group. The prevalence rose progressively from 0.1 per cent below age 40 to 8 per cent in the 60 to 64-year-old men. Occlusions were not infrequent in young men; 1 per cent was noted in the age group 40 to 44.The history was not found reliable for detection of peripheral arterial occlusions. About two of three subjects with demonstrated occlusions failed to exhibit classical symptoms of reduced blood flow in skin or muscle. Conversely, approximately one of four men and one of three women with patent arteries complained of some discomfort in the legs suggestive of occlusions of peripheral arteries.Evidence of general arterial disease was already present in many subjects with peripheral arterial occlusions: 41 per cent had an occlusion in more than one extremity, 27 per cent presented symptoms of coronary heartdisease, and 90 per cent had arteriographic evidence of atherosclerosis in nonoccluded arteries of pelvis and extremities.A similar sex ratio, age distribution, and the frequent concurrence of occlusions of peripheral arteries and coronary heart disease in the same subject strongly suggest a common process responsible for both diseases.A thorough examination of the readily accessible peripheral arteries by methods within the reach of most physicians may lead to the early detection of asymptomatic atherosclerotic disease, permitting the institution of preventive or therapeutic measures before the appearance of myocardial infarction, gangrene, or cerebral vascular accidents.

Correlation of clinical and chemical changes in patients with uremia has not been possible for substances other than urea and electrolytes. Since clinical improvement is frequently seen following hemodialysis, examination of the dialysate for substances of possible importance seemed warranted. With use of the Kiil artificial kidney, small volumes of dialysate have been obtained and found suitable for analytical studies. Amino acid analyses performed after salt removal revealed a preponderance of acidic amino acids and 35 to 38 unknown ninhydrin reactive peaks. Following acid hydrolysis, the unknown peaks decreased in height and known amino acids increased, indicating a degradation of peptides or conjugated amino acids. Elution from the internal volume of Sephadex G-25 columns suggested molecular weights of less than 4000. Confirmatory evidence of low molecular weights was obtained in vitro by estimation of ninhydrin and Folin-Lowry phenol reactive materials remaining in the dialysant compared to glucose and urea. These results suggested that besides free amino acids, the nitrogenous compounds in the dialysate include conjugated amino acids or very small peptides. Of additional interest in one patient was the decrease in removal of some amino acids and other nitrogenous materialsfrom the first to the eighth clinical dialysis, suggesting a relationship to progressive malnutrition observed in this case.

Corrected orthogonal electrocardiographic records were taken from 510 normal male subjects. Their age ranged from 19 to 84 years. Data processing and analysis were performed by means of a digital computer. Measurements were taken from the three scalar orthogonal leads and from a variety of spatial vectors in a Cartesian reference frame. Main emphasis was put on series of instantaneous vectors of the QRS complex, the ST segment, and the T wave. In order to eliminate inter-individual variability in electrocardiographic wave durations, time normalization was applied by dividing QRS, ST, and T into equal parts in time. Results for instantaneous vectors were also obtained in absolute time at intervals of 0.01 second. In addition, time integrals, polar vectors, and Eigenvectors were determined for P, QRS, and T. Normal ranges were computed for both normally and abnormally distributed results.

The vectorcardiographic findings in 29 patients with mitral stenosis are presented.Five patients had normal QRS loops, and five had classical loops of right ventricular hypertrophy with predominant anterior forces. The remaining 19 patients had posteriorly oriented loops with normal rotation. The characteristics of these latter loops were analyzedin detail and compared with the findings in series of normal subjects.The midportions of the horizontal plane QRS loops were found to be significantly more posterior in orientation. Posterior voltage was increased while maximum QRS voltage was decreased.It was thought that the findings represent early right ventricular hypertrophy, and quantitative criteria to differentiate these loops from normal are suggested.

As part of an inclusive health examination of the population of Tecumseh, Michigan, electrocardiograms were recorded from 8,641 persons of all ages, 87.9 per cent of those living in the study area. Eighteen persons, twelve men and six women, had complete right bundle-branch block and a like number, composed of six men and 12 women, had left. All affected persons were past 20 years of age and 67 per cent were past 60.Twenty-five of the 36 persons had no clinical evidence of valvular heart disease, angina pectoris, or myocardial infarction.Bundle-branch block occurred mostly in the older age groups where there was a high prevalence of hypertension, hypercholesteremia, hyperglycemia, and obesity, although these conditions were no more frequent among the affected persons than among others of the same age and sex.
