摘要:
Ninety-eight popliteal aneurysms are reported that occurred in 82 patients who were treated surgically during a 16-year period from 1948 to 1963. The presence of acute or chronic arterial occlusive symptoms prior to operation significantly influenced the results of surgical management. Arterial reconstruction salvaged more than one half of the limbs presenting with limb-threatening ischemia, but early and late results recommend excision and grafting of all asymptomatic popliteal aneurysms before such complications develop. In our opinion saphenous vein autografts are preferable to prosthetic replacement in the surgical management of popliteal aneurysms.

Splanchnic hemodynamics were examined in 22 patients with heart disease, 16 of whom had evidence of ventricular insufficiency at the time of study. The response of the splanchnic vasculature to the exhibition of intravenous digoxin was also studied in nine of these subjects.Ventricular insufficiency was associated with splanchnic vasoconstriction proportional to a generalized increase in peripheral vascularresistance. Splanchnic blood volume was disproportionately increased with respect to total blood volume in patients having visceral congestion with right ventricular failure and combined ventricular failure. A significant relationship between central venous pressure and splanchnic blood volume was demonstrated in these cases. The greater elevation of the former than of the latter suggested the presence of splanchnic venoconstriction.The vascular readjustment to digoxin resulted in a relative intensification of the already existing vasoconstriction of the splanchnic bed as compared to the diffuse systemic vasodilatation which occurred at the same time as the consequence of the inotropic action of the drug: estimated splanchnic blood flow and splanchnic blood volume diminished at a time when systemic flow rose and peripheral vascular resistance decreased. The ultimate distribution of the volume of blood translocated out of the splanchnic bed during this process remains to be determined.

A technic is described for controlling the heart rate in patients with normal atrioventricular conduction by means of an electrical pacemaker catheter that stimulated the right atrium. When the heart rates of 17 patients in the resting state were elevated from an average of 80 to 121 beats/min., the cardiac indices remained virtually unchanged and averaged 3.67 and 3.72 L./min./M.2, respectively. Further increases in the heart rates resulted in small reductions in the cardiac indices to an average value of 3.21 L./min./M.2at 148 beats/min. The stroke volumes, ejection periods, and mean rates of ejection decreased as heart rate was increased.The role of heart rate in the circulatory response to exercise was examined in seven patients. When the heart rates were controlled by electrical stimulation at rates comparable to those previously achieved spontaneously during exercise, it was observed that cardiac output rose normally with exercise and that this rise was accomplished entirely through an increase in the stroke volume. In five patients the effects of isoproterenol infusion were also studied before and during control of the heart rate at the level reached spontaneously during isoproterenol administration. Again, when the heart rate was not permitted to rise, the increases in cardiac output with isoproterenol were mediated through increases in the stroke volume. These studies indicate that in the absence of augmented metabolic requirements, homeostatic mechanisms maintain cardiac output relatively constant despite large induced changes in the heart rate. However, when metabolic demands are increased by muscular exercise, or the circulation is stimulated by catecholamines, cardiac output can rise through an increase in stroke volume, even when alterations in the heart rate are prevented.

摘要:
A new system for recording intracardiac and intravascular sound during routine catheterization is described.This system is based on the application of a flat, sonar-type, barium titanate microphone at the end of the catheter, so that the vibrations of the blood within the chambers of the heart and vessels are transmitted along the fluid in the catheter, and transduced by the microphone (fluid column transmitted phonocardiogram-FCT phonocardiogram).Comparative studies between the response of the FCT system and other systems for intracardiac phonocardiography are made in normal dogs and in dogs with experimental aortic lesions. It is demonstrated that the FCT system gives a good response for vibrations between 25 and 400 cycles per second.Representative phonocardiographic tracings in patients with congenital or acquired heart disease during routine cardiac catheterization are presented.

Red blood cell survival was determined in patients with aortic valvular disease, postoperative patients with aortic valvular ball-valve prostheses and postoperative patients with multiple ball-valve prostheses. The red blood cell survival was reduced in the majority of patients in each group when compared with the red blood cell survival from a normal control group.A detailed analysis of the survival curves suggested that in many patients there was more than one population of red blood cells. The first population displayed rapid random destruction. This population was not present in normal persons in the control group. The second population showed the usual decline in radioactivity due to random destruction and loss of the red cell label due to elution. The shortened red blood cell survival in some patients was due to a large percentage of the first population of randomly destroyed red blood cells, in other patients to an accelerated rate of destruction of the usual single population of cells while others had a combination of the two mechanisms.A mechanism of mechanical hemolysis due to increased intracardiac turbulence was suggested as a cause for the shortened survival. When the turbulence was increased by a leak around the aortic or mitral valve prosthesis the red blood cell survival was found to be further decreased. In some cases this reduction in survival was enough to produce hemolytic anemia.The Coombs antiglobulin test was positive in three patients. The suggestion was made that the development of autoantibodies to red blood cells was secondary to increased destruction of red blood cells.

摘要:
A case of an anomalous arteriovenous-like communication between the right ventricle and a single right coronary artery with pulmonary atresia and an intact ventricular septum in a 6-year-old boy is presented. The dynamics of blood flow through the anomalous vessel are demonstrated for the first time in the literature by means of cineangiocardiography. Factors affecting the flow of blood through the vessel, theories of the embryologic etiology of the condition, and clinical features allowing diagnosis during life and determining therapeutic management are discussed.
