摘要:
Complementary and alternative therapies have been accepted and used in perinatal nursing and midwifery practice for decades. Many authors describe a need for scientific scrutiny of these modalities to determine their efficacy and usefulness in patient care and to create some comparability among studies. The article presents a selective review of the current and readily available English literature about complementary and alternative therapies in perinatal care. Although there is a substantial amount of information published about these modalities in perinatal care, much of it is anecdotal. With the advent of definitions, descriptions, and funding in the United States and abroad, it is likely that additional research of improved design and comparability will be forthcoming.

The Human Genome Project (HGP) and clinical trials of gene therapy create an expanded role for genetic counselors, nurses, and other health care practitioners. Translating fundamental advances in genetics into a benefit of clinical practice can be challenging. In addition to patients, the public and other professionals, including judges and lawyers, need to be educated about genetics and genetic testing and the potential for genetic intervention. The article provides an overview of the HGP, explains the concept of gene transfer, provides examples of clinical trials for specific diseases, and addresses practical issues related to the role of the coordinator in clinical trials of gene transfer.

Since the discovery of the ability to detect fetal heart tones in the 1980s, there has been ongoing research about how to interpret the meaning of these data. Fetal pulse oximetry, also known as fetal oxygen saturation monitoring, is a new method of assessing fetal status during labor that is under investigation in the United States. The article discusses limitations of current methods of fetal assessment, the potential benefits and limitations of fetal oxygen saturation monitoring, and future possibilities for fetal assessment during labor.

The practice of providing developmentally supportive caregiving is familiar to nurses but often is not practiced until after the infant's stabilization in the neonatal intensive care unit (NICU). The study reported here explored the constructs that describe the characteristics of physiologically stable extremely low-birthweight infants and the process of caregiving to these infants. A purposive sample of 20 experienced nurses in a level m NICU was selected. Two separate constructs emerged: individually defined stability and being-in-tune caregiving.

Blood transfusion therapy plays an important role in the care and management of ill newborns, yet the indications for blood product administration remain controversial. The article reviews basic definitions, indications for blood product transfusion, and risks and difficulties involved in transfusion therapy. Descriptions, indications, ABO/Rh compatibility, shelf life, necessary equipment, dose, rate, and general considerations for various blood product components are included.

摘要:
The objective of this research was to learn more about nurses' perceptions of the sources of stress in a neonatal intensive care unit and based on the results, to initiate programs to combat stressors. Nurses were asked to complete a demographic sheet and stress audit. Thirty-three (52%) nurses responded and reported that their greatest source of stress was centered on bio-medical equipment — specifically on access, storage, design, and function issues. Nurses reported their least sources were advanced positions, stable infants, and routine procedures. Plans are underway to remedy equipment issues — primarily to inform and collaborate with others.
