Medicine, from the point of view of its use of technological innovation, is considered by the author to be the most underdeveloped activity world-wide. Present-day engineering expertise can plan, design and construct a great variety of devices and techniques which increase the effectiveness of medical diagnosis and of treatment, of rehabilitation of the disabled, together with a range of‘spin-offs’-for example assisting in producing improved sports performance.The difficulties in exploiting to the full the opportunities available to engineers in medicine are much the same as those which inhibit the exploitation of engineering in general to its full potential. These difficulties, according to Professor Kenedi, are not the result of the oft reputed‘lack of liaison’between technological education and industry (the latter incidentally being the health service for engineering in medicine) or of the apparent‘lack of orientation’of the UK's young people to engineering-these are only symptoms. The malaise is considered to be deeper seated and more subtle: although living in a characteristically multiprofessional world (this is particularly true of the health service) the British still attempt to guide the development of engineering on the bases of inapplicable, restrictive and inhibitive uni-professional concepts and principles.For example Professor Kenedi suggests that the UK should start to register, statutorily, qualified engineers' pre-experience so as to compile a reliable data-base on which to plan manpower, and engineers should not be‘chartered’for life in a particular professional stream just because institutions are uni-professional. Individual career reorientations are now commonplace and an engineer's original‘chartering’is frequently inapplicable in later career. In this context, reaccreditation should be employed, firstly to make‘chartering’keep pace with career changes, and, secondly, (more importantly) to ensure periodic updating of competence. Industry is now acutely conscious of the increasingly frequent incidence of‘skill obsolescence’, particularly in the ranks of its highly qualified staff.In the UK's Engineering Council there is the organization to guide, encourge and, when necessary, push engineering out of its present ineffective morass. To do this with effect the Council will need to adopt a directly active altitude to the introduction of reforms that are so very much needed now.

A patient-controlled analgesia (PCA) system is described. The system based on the Apple He microcomputer linked to an Imed 929 computer controllable infusion pump. The system provides a method for the collection, storage, plotting and analysis of data for PCA research.

摘要:
A microcomputer keyboard substitute for use by the disabled is described. The system displays the characters of a standard keyboard as a matrix where each character is enunciated by an LED. The matrix of characters is scanned by two simple switches until the desired character is reached. This is then entered into the computer using one of the two switches. The system is designed for use on the BBC Model B microcomputer but is relatively easily adapted for use on other machines. It operates in parallel with the conventional keyboard and does not depend on additional system software. It does not use any of the standard computer interfaces, and the character grid can be changed to suit the needs of a particular user.

For some years, external fixators, in which screws or pins project from the fixator (which, as its name suggests, is outside the limb) into parts of a fractured bone to hold them firmly together, have been used successfully to hold fractures in long bones of the arms and legs, and also fractures of the pelvis and jaw [1]. In many instances, this method has advantages compared with Plaster-of-Paris or internal fixation with metal plates or rods.

摘要:
The Nutromat Pfrimmer pump, (shown in figure 1), has recently been introduced to the UK market. Like other enteral feeding systems it has to be used with its own specially designed administration set. The pump can operate from either a 240 V mains supply or its own internal battery pack. A battery-charging unit is provided with the pump.

A true r.m.s. reading rf ammeter utilizing low-cost integrated circuits has been developed for use in measurements of the power output and rf leakage of electrosurgical units. The meter is sufficiently accurate for most clinical applications without the drawbacks of the thermal ammeter.

As part of the continuing evaluation programme of medical equipment sponsored by the UK Health Departments, the evaluation of lung ventilators is being undertaken by the University of Wales College of Medicine. The technical performance and safety assessments are being made within the Department of Anaesthesia and the clinical trials carried out within the South Glamorgan Area Health Authority. Testing to BS 5724 is done at the British Standards Institution.‘Health Equipment Information’Number 143, published in June 1985, contains evaluation reports on a further four instruments intended for use with infants. These are the East Health Care Sheffield Mk 4, the Hoek Loos AIVMk3, the Draeger Babylog 1 and the SLE Newborn.For full details of the evaluation findings, readers should consult‘HET 143–the following are extracts from the report.

As part of the continuing evaluation programme of medical equipment sponsored by the UK Health Departments, the evaluation of defibrillators is being carried out within the Sheffield and Rotherham District Health Authorities.‘Health Equipment Information’(‘HEI’) Number 146, published in July 1985, contains evaluation details on a further three defibrillators. These are the Cardiac Recorders 280/45, the Honeywell ED 420, and the Nihon Kohden Cardiolife TEC5200B.Earlier summaries of evaluation reports have appeared in‘Journal of Medical Engineering&Technology’, together with a review of defibrillator design and development [1 and 2].
