Factors associated with decisions to enter careers in clinical research were studied based on surveys completed by successful clinical investigators. Three aspects of career decision-making were explored: the timing of the decision to enter a research career, the relationship between research success and the timing of the decision, and the impact of two disincentives to enter research careers (the payback provisions of the national research service awards and the abolition of the physician draft that decreased the attractiveness of research training as an alternative). Survey results document the importance of medical school experiences in career decisions. Further, the correlation between the time of career decision and research success supports arguments for early research experiences upon which decisions can be made. Disincentives for entering a research career were not seen as barriers that would have prevented the majority of respondents from entering a research career. The results of the study led to a strong recommendation that research opportunities for medical students be enhanced as one mechanism for positively influencing decisions in favor of research careers.

This paper reports the results of a survey to determine the extent to which interdisciplinary health team concepts are being taught as part of the undergraduate curriculum in U.S. and Canadian medical schools. Less than a third (29 percent) of the respondents indicated the existence of a formal program or program component designed to teach health care team skills. Although the total number of reported programs is small, 53 percent of these programs were required for all students. Three overall categories of content taught within the programs included group process skills, role and function of the team and its members, and content related to the health care context. Although the team approach is a process orientation, fewer than half of the programs reported using direct application of the process, that is, building groups of students into teams.

Individuals occupying the position of chief medical resident have the opportunity to influence significantly the quality and tenor of medical education and medical practice. To assess the status of the position, the authors surveyed chief medical residents completing their tenure in 1980. These residents distributed their time among administrative (41 percent), teaching (35 percent), patient care (21 percent), and research (3 percent) activities. They reported that their experience was quite positive. However, overall satisfaction was significantly negatively correlated with percentage of time spent performing administrative tasks. The percentage of time chief medical residents allocate to administration has increased during the past decade, while time spent teaching and delivering patient care has decreased. Based upon this survey, recommendations for improved utilization of the time and talents of these individuals are presented.

The 1970s have been described as an era of the rise and fall of three-year medical school programs. The present paper describes how graduates of one of the few North American medical schools that has retained its three-year curriculum view the problem-based curriculum and its length. On the whole the graduates of McMaster University School of Medicine were very positive about their undergraduate medical education. Many more of its features were endorsed as strengths than deficiencies. Eighty-two percent of McMaster graduates reported they would enroll in a three-year curriculum if they were entering medical school again, and they said that the advantages of a three-year curriculum outweight its disadvantages. The majority of graduates would return to a problem-based medical curriculum: 58 percent to an unaltered curriculum, 3 percent to one altered slightly, and 8 percent to one spread over four years. Of those that would not return, most chose a somewhat more structured curriculum, while very few chose traditional curricula.

With the advent of low-cost modular microcomputer technology, discussions of computer-based instruction (CBI) have taken on a new direction. Earlier efforts at computer-based instruction were successful in the technical area, but the expense of starting and maintaining the programs deterred the widespread application of CBI. The initial low purchase price and minor maintenance expenses of microcomputers have lessened financial concerns about equipment purchases. The smaller size and the capability of the microcomputer have allowed CBI system developers to focus on more specific (rather than monolithic) projects so that the time and the cost for program development have been reduced. Although students have demonstrated high interest in CBI, faculties expressed some reservation about entering the field because of the new technology and the lack of supporting evidence for the value of this teaching modality. The microcomputer may be the answer to the economic and attitudinal deterrents to the success of previous attempts at CBI.

摘要:
An economical, computerized system was developed for producing the annual schedule of monthly assignments for medical house officers. This program can generate schedules that conform to the individual preferences of the house officers while meeting the requirements of the residency program. As a result, more individual requests of the house officers are granted than under the previous manual system. Besides providing these direct benefits, the system ensures that house officers are assigned to each rotation according to their appropriate level of skill.

摘要:
The teaching of respiratory therapeutics in medical schools has lacked structure and cohesiveness. Comprehensive, formal programs which address the clinical needs of fourth-year students in the areas of acute and non-acute respiratory therapy problems have not been reported in the literature. A report on a pilot program in respiratory therapeutics, including a development and implementation model, is presented. Prerotational and postrotational evaluations are described. Significant improvement in specific competency areas related to clinical skill areas is also reported.
