In this paper, the author describes the results of tracking a cohort of medical school graduates through the graduate medical education system of Ontario, Canada. The cohort consisted of the 1977 graduates of the five Ontario medical schools who entered the graduate system at some point between 1977 and 1982 and graduate trainees who had graduated in 1977 from medical schools outside Ontario. The objectives of the study were to determine what proportion of the total number of 1977 Ontario medical graduates received graduate training in Ontario; to document the flow of the trainees into and out of the Ontario graduate system; and to compare the graduate training patterns of the Ontario graduates in the cohort with the trainees from elsewhere.

History-taking and physical examination techniques specific to clinical evaluation of the pediatric patient can be taught to medical students through a combination of audiovisual materials and limited bedside teaching sessions. In the study reported here, the authors found this combination of instructional methods to be at least as effective as and, in terms of faculty time, much more efficient than a preceptor-type model formerly used at the authors' school. Integration of this instruction into already existing physical examination courses, when student attention is particularly focused on understanding and refining clinical technique, is highly desirable. It also permits more time to be spent in disease-oriented teaching during the required clerkships than would be possible if basic techniques specific for the specialty area were not introduced until the beginning of each rotation.

A prospective study using two standardized psychological tests, the Profile of Mood States (POMS) and the Self-Rating Depression Scale (SDS), was conducted in an effort to quantify the emotional changes experienced by internal medicine house staff members during the internship. In contrast to instruments used in previous investigations of this type, the POMS and the SDS are standardized tests with proven reliability and validity. The six mood factors measured, “tension-anxiety,” “depression-dejection,” “anger-hostility,” “vigor-activity,” “fatigue-inertia,” and “confusion-bewilderment,” are reported to be among those factors most often affected by the internship experience. Twenty-three interns completed both tests at four-month intervals during one academic year. One-way analysis of variance for repeated measures revealed that the level of only anger-hostility of the mood factors changed significantly during the year. The intensity of this factor increased between the first and third testing periods before dropping at the end of the year. In contrast to findings in previous studies, the depression and fatigue factors did not increase during the year. By characterizing interns' reactions to the stresses of postgraduate medical education, standardized psychological tests can contribute to improved understanding of these reactions and to more intelligent planning of support systems.

The clinical experience of 40 Duke University medical students during their required two-month clinical clerkships is analyzed to compare experience on the traditional internal medicine, surgery, pediatrics, obstetrics-gynecology, and psychiatry clerkships with experience on a new family medicine clerkship. On the traditional clerkships, the students saw mostly hospital inpatients for an average of 36 different patients for 142 encounters and 300 problem contacts during a two-month rotation. On the family medicine clerkship, they saw mostly office ambulatory patients for an average of 184 patients for 236 encounters and 416 problem contacts. Traditional clerkships provided the most experience with neoplasms, blood problems, mental illness, obstetrical, perinatal, and congenital problems. The family medicine clerkship offered the most experience with circulatory, respiratory, digestive, neurological, musculoskeletal, and skin problems and with injuries. The family medicine clerkship contributed a substantive and important dimension to the clinical curriculum beyond that offered by the traditional five clerkships.
