A study was conducted by the authors that examined the validity of the Medical College Admission Test (MCAT), undergraduate grade-point average (GPA), and “competitiveness” of undergraduate college in predicting the performance of students at a predominantly black college of medicine. The performance measures used in the analysis consisted of course grades in all four years of medical school and scores on both Part I and Part II of the National Board of Medical Examiners (NBME) examinations. The predictive validities of the MCAT scores and undergraduate GPAs were found to be similar to those revealed in earlier studies conducted at predominantly white schools. Two exceptions to these similarities were found. First, the MCAT scores at the black school had a somewhat lower validity in predicting NBME examination scores than was the case at the other schools. Second, of the six MCAT subtest scores, Skills Analysis: Reading had the highest correlation with first-year grades, in contrast to results at the other schools. No differences between men and women were found in the validity of MCAT scores and the GPA. The competitiveness of the undergraduate college attended was found to contribute significantly to the prediction of all measures of medical school performance.

The purpose of the study reported here was to determine the relationship between Scholastic Aptitude Test (SAT) scores and Medical College Admission Test (MCAT) scores of black students who had participated in a summer program for minority students interested in health careers held at the Medical College of Georgia. A significant correlation was established between these students' SAT and MCAT scores. These findings suggest that students who are likely to perform well in medical school can be selected for medical school while they are still underclassmen, accepted conditionally, and admitted after completing a prescribed curriculum. Early selection has implications for increasing the number of minority students in medicine and for permitting more liberal arts in the premedical education than the present selection process.

Few studies that examine the career decisions of medical students have been based upon theories of decision-making. Several theories were used by the present authors to study differences among first-year medical students in North Carolina who preferred family medicine and those who preferred other specialties. Responses by 358 first-year students to a career preferences questionnaire administered in the fall of their freshman year revealed that students who preferred family medicine were more interested than other students in using medicine as a tool to help people. They were equally persistent and thorough in their career decision-making, were at an earlier stage of decision-making, and had less concern with themselves compared with other students. Decision-making theory helps to explain the relationships found in earlier studies between students' characteristics and their specialty preferences. The results of the present study also may explain the reason for the decrease during medical school in the number of students who prefer family medicine as a career. Those results show that first-year medical students who prefer family medicine are at an earlier stage of the decision-making process than other first-year medical students.

A study of stress during residency training involved data analysis of questionnaires completed by 108 residents and fellows in internal medicine. Over 40 percent of the respondents experienced important problems with their spouse or partner. Of these, 72 percent believed that these problems were due to the residency, and 61 percent reported that their spouse or partner agreed with this assessment. Only 21 percent of the residents with relationship problems felt that their hospital work was being affected negatively. A multiple regression analysis showed that 10 variables accounted for 50 percent of the variance in predicting relationship stress. The results suggest that scheduling and structural changes in residencies are necessary in order to reduce stress among residents. Perhaps equally important is the finding that stress can be buffered by family relationships and social contact. The authors conclude that social support systems need to be fostered during the residency.

摘要:
The formal teaching of medical ethics is a recent addition to the curricula of U.S. medical schools. In this paper, the author examines the evolution and goals of teaching medical ethics, the nature of medical ethics, and integrating such teaching into the curriculum. The author concludes that teachers of medical ethics must impart knowledge that can be applied to the problems medical students face. Because moral considerations are as much a part of medical decisions as technical considerations, this teaching is best done in the context of real cases with which the student is involved and in which ethical considerations can be shown to make a critical difference. A brief description of such a teaching program is included.

In all medical specialties, the chief resident position is considered an honor that is the culmination of a resident's hard work and clinical acumen. However, the position requires managerial skills that residents have had little exposure to during medical school and postgraduate medical training. In the paper presented here, the author describes a model involving a medical school department of psychiatry and affiliated residency programs and discusses how various components of the structure are organized (affiliated hospitals, groups of individuals, and administrative hierarchy) and how the movement of information is influenced by organizational structure and interpersonal relationships within the model. The author also describes how information management is a major administrative function for the chief resident and how various predicaments that a chief resident faces stem from his mishandling information and from a misinterpretation of his power and influence. Training issues that could maximize the chief resident's theoretical and practical administrative experience are discussed.

The use of reciprocal peer teaching (RPT) was evaluated by 66 first-year medical students participating in gross anatomy laboratory sessions at the University of Ottawa Faculty of Health Sciences. In RPT each student in turn served as a teacher of his peers. The students compared RPT with staff teaching, and almost all reported that they acquired as much knowledge of topics taught by their peers and greater knowledge of topics that they taught personally than they acquired from staff teaching. In addition, most students reported improved study habits, better attitudes toward anatomy, and more independent study. A majority of the students felt they would be better able to communicate with patients and felt more like students of medicine than before participating in RPT. The authors concluded that the active involvement of students in teaching will help prepare students to be continual and independent self-learners throughout their professional careers.
