Clinical studies of hypertension suggest that a proportion of patients initially diagnosed as hypertensive may actually have normal blood pressure (BP). We have compared automated repetitive BP measurements (each 7.5 minutes for periods of two hours and 24 hours) with conventional duplicate sphygmomanometer readings in the evaluation of 50 patients previously diagnosed as having mild hyper tension. For the group as a whole, the BP values for the conventional readings and for the averages of the two-hour monitoring periods were closely similar for both systolic and diastolic measurements. However, in only 20 of 3] patients (65 %) determined to have systolic, hypertension (±140 mm Hg) by the conventional readings, was the hypertension confirmed by the two-hour method; for diastolic hypertension (±90 mm Hg). it was confirmed in only 24 of 3i patients (71%). Eleven of the 50 patients (22%) were normotensive by both the systolic and diastolic criteria. Data from the full 24-hour BP monitoring periods confirmed, the findings of the shorter-term two-hour studies. Fourteen of 58 healthy volunteers previously shown to be normotensive had a diastolic BP greater than or equal to 90 mm Hg when a further conventional reading was obtained, but 11 of these individuals then had normal BPs during a subsequent two-hour study. Conventional BP measurements, even when correctly performed, may erroneously indicate a diagnosis of hypertension. Short-term automated repetitive measurements appear to provide a reliable and practical approach to minimizing this problem.

This paper examines the nature of social support and social network variables and their relation to coronary heart disease (CHD). In the first section, the dimensions along which social support varies (emotional /material, manifest/potential, perceived/objective, quantity/quality) are discussed. Several social network characteristics that have been found to be associated with health are also examined. Next, the evidence for a relationship between social support and social networks and CUD is reviewed. Those aspects of social support and social networks that appear most likely to be involved in the prevention of CUD and rehabilitation from heart attacks are defined and evaluated. The most promising variables are found to be the number and intimacy of social ties; satisfaction with social activities, work, and financial status; perceived opportunity for discussing problems; and level of social interaction. Further research on those variables mid their relationship to CHD is recommended.

Individual cardiac rehabilitation trials generally have not demonstrated significantly lower reinfarction rates, although there is a trend toward lower fatal events. In a progressive disease such as coronary artery disease, for which there is no known cure at the present time, reduction of disability and handicap may he more appropriate outcomes than a reduction in mortality or morbidity A reduction in disability and handicap may he equated with an improvement in quality of life. Patient self-responsibility for compliance with long-term health-behavior change is an essential factor in regaining an active, productive life and improving quality of life.
