It is the policy of the editors of the Journal of Cardiopulmonary Rehabilitation tokeep its readers up to date with new policy statements and position stands from other professional organizations that are relevant to its readers. In 1978 the American College of Sports Medicine (ACSM) published a position stand entitled “The Recommended Quantity and Quality of Exercise for Developing and Maintaining Fitness in Healthy Adults‘’ which was later reprinted in JCR 1981;1;375–384. The revised position stand printed below was recently published by ACSM(Med Sci Sports Exerc 1990;22;265–274.) and replaces the 1978 statement. Although the statement is related to the healthy adult, its reference to the elderly, low fit, or obese person makes it relevant for wellness and cardiopulmonary rehabilitation programs. Of particular interest is the classification of intensity of exercise based on 20 to 60 minutes of endurance training (Table I). This classification system is valid and practical for use with patient populations as well as with elderly and low fit participants. An important addition to the present statement includes a resistance-training component to the training program. The importance of a well-rounded program is emphasized with the 8 to 10 exercises recommended to train the major muscle groups. The statement notes that such exercise is very specific. Thus, training the legs will have little or no effect on the arms, shoulders, or trunk. The editors believe that the new ACSM position stand is important to read because most health professions involved in rehabilitation treat a variety of patients, many of whom could benefit from this statement.

Compared to routine care, cardiac rehabilitation programs can produce a number of substantial benefits for patients after their cardiac event. Programs that include exercise can improve morbidity, both physiological and psychological, and probably can improve mortality rates. Programs of psychotherapy or psychosocial therapy also can produce numerous improvements in physiological and psychological morbidity, but improvements in mortality are unproven. Long-term benefits from purely educational programs have been very modest. Results from the several good studies of comprehensive, multifactorial programs are impressive. In administering cardiac rehabilitation programs, it is important to consider coordination of services, education after discharge, inclusion of other health professionals in addition to physicians, and selection of patients to rehabilitate.

A survey of 202 cardiac rehabilitation programs randomly selected from the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) mailing directory was conducted to investigate the current role of psychosocial services in cardiac rehabilitation. The 117 programs responding to the survey consisted primarily of hospital-based programs providing both inpatient and outpatient services and was directed by individuals with nursing backgrounds. Program directors with nursing background tended to view psychosocial services as somewhat more valuable than did directors with exercise physiology backgrounds. Approximately two-thirds of these programs performed some type of psychosocial assessment, most commonly administered by a nurse or an individual with several academic credentials. Assessments tended to emphasize clinical interviews and unstandardized questionnaires, while instruments with higher psychometric qualities were used less frequently. Most programs incorporated some form of service addressing the patients' psychosocial needs. Such programs typically included involvement of the patient and family in planning the rehabilitation process as well as the use of a stress management or patient support group. Individual or group psychotherapy for the individual with special psychosocial needs was available in only a small percentage of programs. Low or inadequate third party reimbursement policies was perceived as adversely influencing the availability of psychosocial services, particularly assessments. The discussion focuses on the need to use assessment instruments with documented psychometric properties, particularly for depression, and conceptual models for integrating psychosocial services into cardiac rehabilitation programs.
