Changes in exercise endurance were evaluated in a group of 57 patients with mild to severe chronic obstructive pulmonary disease (COPD). The subjects all participated in an 8-week pulmonary rehabilitation program. The dose-response relationship between compliance with walking prescriptions and change in exercise endurance was studied. Compliance was a significant predictor of changes in exercise endurance from baseline to the end of the 8-week treatment. Multivariate analysis suggested that this relationship was not mediated by level of disease severity or self-selection. Other variables, including measures of lung function, depression, quality of well-being, social support, and smoking history, did not significantly predict changes in exercise endurance. It was concluded that exercise compliance is an important factor in pulmonary rehabilitation and that the treadmill walking test of exercise endurance is an important outcome measure because it closely approximates the behavior, (i.e., daily walking) for which patients are being trained.

This study was designed to develop an equation for predicting oxygen uptake (VO2) from treadmill performance in a group of patients who have had coronary artery bypass grafts (CABG) and were referred for cardiac rehabilitation. Ninety-nine post-CABG patients underwent precardiac and postcardiac rehabilitation Phase II exercise testing using the Bruce Protocol with collection of expired air. The following variables were employed in the model: total treadmill exercise time (Time), age, weight, sex, presence and/or absence of medications, and an indicator variable period that was used to label observations as either pretraining or post-training. A linear multiple regression equation using the above variables hasR2= 0.701. Other commonly used equations were found to overestimate VO2in the range from 4% to 250% with average error of 75%. Our results indicate that direct measurement of VO2is essential when precise measurements of functional capacity are required. In less demanding settings, the proposed regression model can be a very useful tool for predicting VO2in patients who have undergone CABG.

This study was undertaken to determine the restoration of leisure and social activities and vocational status in the first year after myocardial infarction. In addition, the contribution of cardiac rehabilitation (CR) was evaluated. For this purpose, a test for leisure and social activities (LSA) was developed. Demographic, medical, and psychosocial characteristics of 366 consecutive patients who had a myocardial infarction were used: 265 patients who underwent CR, 50 patients who refused CR, and 51 controls randomized to no CR. Questionnaires were filled out at 3 weeks, 3 months, and 1 year after discharge. The LSA was shown to be a simple and valid test. Mean LSA of the 366 patients improved from 75% at discharge to 90% at 3 months (P< 0.0001), and to 95% after 1 year (The difference between mean LSA at 3 months and 1 year was not significant.) Men did better than women; younger patients (less than 55 years old) improved more than older patients (55 to 70 years old). Lower LSAs were observed with higher anxiety levels; increase of LSA over time, however, was independent of degree of anxiety at discharge. Vocational status did not relate to the LSA. Multiple regression analysis indicated that return to former LSA was predicted by magnitude of LSA at discharge; in addition, high anxiety and older age predisposed for lower LSA during follow-up. The effects of CR were studied by comparing the 51 randomized control patients with a matched group of 153 patients in the CR group. After 1 year the LSA was better (P< 0.05) in the CR group. Subgroup analysis indicated that CR led to higher LSA in older and single patients and probably also in women and higher educated patients. Vocational status was unaffected by CR.

Exercise training is commonly used in the rehabilitation of patients who have undergone heart transplantation (HT). This study determined the effects of an 8-week to 10-week cardiac rehabilitation program (CRP) on the ventilatory threshold (VT) of orthotopic HT recipients. Eleven men completed an average of 22 ± 4 (± standard deviation) exercise training sessions over 9 ± 2 weeks. Graded exercise testing was performed before (T1) and immediately after (T2) CRP participation. From T1 to T2, absolute peak VO2increased by 0.28 ± 0.05 L/min (± standard error) (P< 0.05). Assessment was made by four independent reviewers. Although absolute VT-VO2L/min increased by 0.21 ± 0.05 (P< 0.05), VT% (VT VO2/peak VO2x 100) did not significantly change. Rate of perceived exertion (RPE) at VT was 13.2 ± 0.33 and 12.8 ± 0.54 for T1 and T2, respectively, and this change was not significant. Relative peak VO2, relative VT, and body weight also did not demonstrate significant changes from T1 to T2. Results suggested that for HT recipients following an 8-week to 10-week CRP started 2.5 months after surgery, the normal exercise training RPE reflected VT during exercise training, the RPE was not significantly changed in relation to VT, and the absolute VO2at VT was significantly increased. Additional research is needed to determine if VT-RPE may be used to guide exercise intensity.
