There is controversy regarding the effectiveness of exercise training in patients taking beta blockers. Therefore, this study was done to compare exercise training in 64 patients with coronary artery disease who were not taking beta blockers (NBBG) and 60 patients who were taking beta blockers (ABBG). Exercise training was done using treadmills and bicycle ergometers for one hour three times weekly for 12 weeks, with electrocardiographic monitoring. The resting heart rate decreased from 76 beats/min to 70 beats/min in the NBBG and from 60 beats/min to 57 beats/min in the ABBG. During submaximal exercise (stage II on the Bruce protocol), the heart rate decreased from 127 ± 15 beats/min to 115 ± 15 beats /min in the NBBG and from 104 ± 13 beats/min to 96 ± 11 beats /min in the ABBG. The systolic blood pressure similarly decreased from 166 ± 24 mm Hg to 160 ± 25 mm Hg in the NBBG and from 152 ± 19 mm Hg to 141 ± 17 mm Hg in the ABBG. The ST-segment depression (V5) during stage II exercise decreased by 22% in the NBBG and by 25% in the ABBG. The maximum exercise capacity as measured by symptom-limited Bruce treadmill test increased by 28 % in the NBBG and by 24 % in the ABBG. These changes are statistically significant. The beneficial effects of exercise training were equally effective in patients receiving beta blockers.

Enalapril, a new angiotensin-converting-enzyme inhibitor, was more effective when combined with hydrochlorothiazide than either drug alone in controlling moderate hypertension over a 16-week period in 20 middle-aged men and women. Fifteen of these subjects underwent progressive treadmill exercise tests at baseline while receiving placebo and after blood pressure control was achieved. Systolic blood pressure response to submaximal and near-maximal exercise was significantly lower during treatment. However, there was no significant effect during exercise on heart rate, perceived exertion, respiratory exchange ratio, exercise test duration, or oxygen uptake.

The practice of denying type A (heavy commercial vehicle) licenses to successfully rehabilitated postcoronary patients is critically reviewed. The present North American policy makes a negligible contribution to the reduction of traffic accidents. Physicians should advocate more effective preventive measures: particularly, a firmer enforcement of legal blood alcohol limits for drivers of all classes of vehicles.

Repeat ventricular function and oxygen consumption (VO2) studies were obtained for ten patients with coronary artery disease (CAD) at rest and during supine radionuclide exercise ventriculography. Additionally, each patient underwent an upright exercise treadmill test so that cardiovascular responses during supine and upright exercise could be compared. Measurements of left ventricular ejection fraction, rate-pressure product (RPP), and VO2either at rest or during exercise were reproducible using supine radionuclide ventriculography tests. Comparison of data between supine ergometry and upright treadmill exercise showed higher blood pressure but lower heart rate and VO2values for supine exercise. There were significant correlations in the two postures for maximum heart rate and RPP during exercise. Since ventricular function and VO2are reproducible using supine radionuclide ventriculography, this procedure can be used not only as a diagnostic test, but for measurement of functional capacity and VO2after a therapeutic intervention. Inasmuch as maximum upright heart rate and RPP can be reasonably predicted from supine exercise testing, supine exercise could serve in certain situations as an alternative method for establishing an exercise prescription.
