Levels of cardiovascular risk factors were determined in 75 patients with Type 2 diabetes mellitus. The patients were divided into three groups according to their urinary protein excretion (UPE): (a) normal proteinuria (≤ 70 mg d−1); (b) microproteinuria (70–500 mg d−1): and (c) macroproteinuria (>500 mg d−1). A significant stepwise increase in mean systolic blood pressure, LDL‐cholesterol and fibrinogen levels was observed from the first to the third investigated group of patients. Mean apoprotein B levels were significantly increased in the group with macroproteinuria compared to the other two groups. Significant linear correlations were found between UPE and LDL‐cholesterol, total cholesterol, apoprotein B, creatinine, systolic blood pressure and diabetes duration. The width of the regurgitant jet at the aortic valve plane, i.e. the core flow diameter, the ratio of the jet width to the left ventricular outflow diameter, the regurgitant volume and regurgitant fraction were determined using two‐dimensional, continuous wave and colour flow Doppler echocardiography. The relationship between the non‐invasive measurements and semiquantitative angiographic grading of the regurgitant flow (1 + to 4 +) was examined in a primary group of 20 patients with chronic aortic regurgitation. Cut‐off points for the non‐invasive measurements were selected so as to separate patients with mild or moderate regurgitation (1 + or 2 +) from patients with moderately severe or severe regurgitation (3 + or 4 +). These cut‐off points were prospectively applied in a new group of 35 patients with aortic regurgitation to predict the angiographic grading. Jet width correctly predicted the angiographic grading in 86% of cases, the ratio of the jet width to the outflow diameter in 83% of cases, the regurgitant volume in 86% of cases and the regurgitant fraction in 91% of cases. Gallbladder (GB) volume was monitored by real‐time sonography in diabetics (n= 21) and healthy volunteers (n= 55) after a test meal. Seventeen controls and seven diabetics were obese; six patients had both autonomous and somatic neuropathy, and four had somatic neuropathy. Fasting GB volume was similar in controls and diabetics with and without autonomic neuropathy; it was correlated with body mass index (controls,r= 0.43,P<0.002; diabetics,r= 0.46,P<0.04), and was increased in obese subjects. Post‐prandial GB emptying was decreased in diabetics. Those with autonomous neuropathy exhibited larger residual volumes than controls (P<0.03). Post‐prandial GB emptying was slower and less complete in (non‐diabetic) obese subjects and deteriorated further in diabetic obese subjects. Multiple myeloma affects bone, and enhanced bone resportion is a characteristic finding. In the present study the serum concentration of osteocalcin, serum bone gla‐protein, which is a protein specific for bone turnover and reflects osteoblast activity, was analysed at diagnosis in 48 patients with multiple myeloma. At that time there was a significant relationship between disease stage (Durie‐Salmon) and osteocalcin levels, lower levels being found in patients with more advanced disease. No relationship was found between osteocalcin and serum calcium levels. To date, 33 patients have died. There was a significant correlation between initial osteocalcin levels and patient survival. We observed a decrease in serum bilirubin, alkaline phosphatases (ALP) and IgM in five patients with primary biliary cirrhosis (PBC) treated with Prednimustin (Sterecyt®) for 6 months. In contrast to pretreatment findings, C3 activation was undetectable during treatment in three patients where normalization of serum IgM was achieved. After discontinuation of Prednimustin, bilirubin and ALP levels rapidly returned to pretreatment values, although IgM remained normal for up to 6 months in some patients. We conclude that Prednimustin might be of value in patients with symptomatic PBC where liver transplantation is not an option, and that it should be evaluated in a controlled study. However, the rapid reactivation of the disease after conclusion of treatment must be considered
