The diagnosis of liver nodules may be problematic for pathologists, information about the clinical presentation and radiologic imaging can facilitate the interpretation of pathologic material. This review discusses the salient imaging features of the more commonly encountered liver nodules. The radiologic findings are correlated with the pathologic features, emphasizing signs that can clarify the diagnosis. An imaging alforithm used to evaluate patients with liver nodules is presented.

摘要:
Although liver fine needle aspiration is widely used, there have been few objective analytic studies measuring true costs and other patient outcomes. For the diagnosis of liver masses, compared to fine needle aspiration, alternative testing strategies generally are viewed as too costly or are untenable for other reasons. Additional studies are needed to validate this belief and to determine the cost-effectiveness of different means of preparation, screening, and interpretation of liver fine needle aspirations in the cytology laboratory.

摘要:
Following fine needle aspiration (FNA) of core biopsy of the liver, a number of therapeutic options are becoming available. For many years hepatic resection has been the only effective treatment for patients with hepatic malignancies. Resection of colorectal liver metastases can achieve a cure in 25 to 45% of patients. Unfortunately only 10 to 15% of patients are eligible for resective therapy, which has led to the development of new and innovative therapies. Initial results with ablative therapies such as cryosurgery, radiofrequency ablation, and alcohol injection have demonstrated response rates of >60% and survival rates of 36 months. The goal is to develop effective ablative therapies that can be used in conjunction with resection, Hopefully, this multimodality approach will increase the number of patients that can be cured of the hepatic malignancy.In patients who are not resection candidates, aggressive therapies are used in an attempt to downstage. Hepatic arterial chemotherapy delivers a high does therapy and produces an objective response in 50 to 72% of patients. The majority of hepatic malignancies have a mutation or deletion of p53 and restoration of p53 leads to tumor cell death. The intraarterial approach is being used to examine the effectiveness of administering p53 adenovirus in patients with hepatic malignancies. This paper will review various therapies for treating and downstaging hepatic malignancies following FNA and core needle biopsy of the liver.
