33 Patienten mit einem neu diagnostizierten Morbus Hodgkin und meist mehreren besonders ungünstigen Risikofaktoren wurden im Dreijahreszeitraum 3/86–2/89 in einer Phase-II-Studie nach dem COPBIAM-Protokoll behandelt. Die mediane kumulative relative Dosis-Intensität (RDI) als Maß für die applizierte Zytostatikamenge pro Zeiteinheit war mit 0,97 sehr hoch. Es erreichten 14 Patienten (43%) eine Vollremission (VR); weitere 11 (33%) wurden wegen kleiner residueller Tumoren im Gebiet ursprünglich sehr großer Tumormasse zunächst als weitgehende Remission (WR), 7 (21%) als Teilremission (TR) und ein Fall (3%) als Progreß eingeordnet. Unter der nachfolgenden Radiotherapie (RT) verkleinerten sich die Resttumoren der als WR eingestuften Patienten nicht weiter, so daß diese Fälle retrospektiv ebenfalls als VR bewertet wurden, die somit nach Abschluß von COPBIAM 76% betrug. In vitro, the combination of interleukin-2 (11–2) with interferon-α (IFN-α) seems to act synergistically on the generation of lymphokine activated killer (LAK) cells. Due to this fact two clinical trials with the combination of 11–2 and IFN-α were initiated in malignant melanoma (MM) and renal cell cancer (RCC). Patients with disseminated MM were treated by a sequential application of 10 × 106 U/m2 rlFN-α2b s.c. on days 1–7 followed by continuous intravenous infusion of 3 × 106 U/m2 rIl-2 on days 8–13 and 15–20. After a pause of 4 weeks the cycle was repeated. In advanced or disseminated RCC, the patients were treated with a daily alternating scheme of 10 × 106 U/m2 rIFN-α and rIl-2 as 1 h infusion 1 ×/day for 14 days. The rIl-2 escalates intra- and interin-dividually beginning with a dose of 3 × 106 U/m2. The cycles were repeated after a pause of 3 and 4 weeks, respectively. The preliminary results show that the schedules are practicable and that the toxicity of the combination of rIl-2 and IFN-α does not accumulate. Thirty-three patients with untreated small cell lung carcinoma received 6 cycles epirubicin 30 mg/m2 i.v. on days 1, 8, and 15, in combination with cyclophosphamide 1,000 mg/m2 on day 1 and vincris-tine 2 mg i.v. on day 1 every 3 weeks. Thirty-one patients were evaluable concerning state of remission. Four out of 13 patients with limited disease had a complete, and 6 a partial remission, while 2 out of 18 patients with extensive disease had a complete and 8 a partial remission. The toxicity of this therapy regimen was very low. The results of this treatment did not differ significantly in comparison to the standardized treatment of small cell lung cancer, but it was easier to tolerate.

In stage III non small cell lung cancer initial tumor size is a neglected prognostic factor. 22 patients with stage III disease have been treated with radiochemotherapy (cisplatin/vindesine). A significant (p < 0,001) influence of initial tumor size on local control can be demonstrated. Future treatment strategies should include initial tumor size.
