The historical development of different techniques for the study of proliferative patterns of normal and malignant tissue is explained. For more than 100 years, pathologists and clinicians have attempted to clarify the prognostic impact of this tumor-biological parameter. One relevant result of these investigations is the ‘Grading-System’, a histomorphological description of tumor cell differentiation. The auto-radiographic method using tritiated thymidine (thymidine incorporation assay) detects a portion of DNA-sythesizing cells directly. However, radioactivity and long autoradiographic exposure times are the main disadvantages. DNA cytophotometry and immunohistochemical labeling are currently the most frequently used techniques for direct assessment of proliferating cells in normal and malignant renal tissue as well as in other human tumor tissues. Concerning renal cell carcinoma (RCC), our own experience and the results of the literature are compared and discussed: In contrast to tumor staging, tumor grading correlates with the tumor-specific proliferation rates. According to the present data, the measurement of the proliferative kinetics seems to be a relevant prognostic parameter for detection of high-risk patients, especially in the early stages showing identical histological findings for staging and grad

In der klinischen Forschung sind kontrollierte, randomisierte Studien heute als die Methode der Wahl akzeptiert, die Wirksamkeit von Therapien zu überprüfen. Die Tatsache, daß eine solche Studie durchgeführt wird, reicht allerdings allein noch nicht aus, einen validen Therapievergleich zu garantieren. Vielmehr muß die Studie zusätzlich gewissen Qualitätsanforderungen genügen. um sowohl die interne als auch die externe Validität, d.h. die Verallgemeinerbarkeit der Resultate, zu gewährleisten. Die Anforderungen betreffen in erster Linie die zentrale Patientenanmeldung und Randomisation in einem unabhängigen Methodischen Zentrum sowie eine der Fragestellung angemessene und nachvollziehbare Anzahl aufgenommener Patientinnen. Eine adäquate Beschreibung des Studienkollektivs ist ebenso unabdingbar zu fordern wie eine Beschreibung derjenigen Patientinnen, die potentiell für die Aufnahme in die Studie geeignet waren. Bezüglich der Behandlung von Drop-Outs muß nach dem «lntention-to-Treat»-Prinzip verfahren werden; Sicherung der Compliance der Patientinnen sowie einer möglichst hohen Datenqualität sind weitere wichtige Punkte, die beachtet werden müssen. Bei der Auswertung müssen die Therapieeffekte und die Effekte von wichtigen prognostischen Faktoren im Rahmen einer multivariaten Analyse simultan berücksichtigt werden; der Angabe von Konfidenzintervallen ist gegenüber der alleinigen Präsentation von P-Werten der Vorzug zu geben. Insgesamt sollten unkontrollierte Zwischenauswertungen sowie eine zu frühe Publikation von Studienergebnissen vermieden werden. Die Qualitätsstandards werden in dieser Arbeit unter besonderer Berücksichtigung von Therapiestudien beim Mammakarzinom diskutiert; sie treffen aber im Prinzip auch auf andere onkologische Studien zu. Combination chemotherapy with etoposide, leucovorin and 5-FU (ELF) was investigated in a phase-II study in patients with advanced gastric carcinoma (stage III and IV). Only ambulatory patients (performance status WHO ≤2) above 65 years or younger patients with cardiac disease were entered. A remission rate of 53% could be achieved in 51 evaluable patients, including 12% complete remissions. One toxic death was observed. Remission rate was particularly high in patients with locally advanced disease (70%, 7/10 patients). Median remission duration was 8 months, median overall survival 11.5 months. Patients with peritoneal carcinosis lived significantly shorter than those with other distant metastases (P < 0.05). ELF was well tolerable. Seven neutropenic patients with the acquired immunodeficiency syndrome (AIDS) or AIDS-related complex (ARC) were treated with recombinant human GM-CSF in an attempt to reverse their leukopenia. Overall, treatment was associated with a rapid, substantial increase in neutrophilic granulocytes and a smaller increment in eosinophils and monocytes. No significant increase in platelets or reticulocytes was observed. Termination of treatment was associated with a rapid drop in the leukocyte count. While some patients with severely hypoplastic marrows did not respond to GM-CSF treatment, azidothymidine- or ganciclovir-induced neutropenia was reversed by concomitant administration of GM-CSF in others. This indicates that rhGM-CSF may be a valuable adjunct to antiviral therapy in ARC/ AIDS and requires further testing in controlled trials.

Von 1986 bis 1990 wurden in der unfallchirurgischen Abteilung des Städtischen Krankenhauses Frankfurt/Main-Höchst 14 Patienten mit Beeinträchtigung der Beckenstatik aufgrund großer Metastasen operativ versorgt. Das Durchschnittsalter betrug 59,9 Jahre. Ein Überwiegen des weiblichen Geschlechts war durch die häufige ossäre Metastasierung des Mammakarzinoms bedingt. Die palliative Operation zielte dabei auf eine Wiederherstellung der Belastbarkeit und der Gehfähigkeit, auf eine Schmerzminderung sowie Rückbildung einer neurologischen Symptomatik. In zehn Fallen waren das Pfannendach bzw. die tragenden Anteile des Os ileum betroffen, in vier Fallen die Ileosacralregion und die Massa lateralis des Kreuzbeins. Es wurde eine sichere, schnelle und simple Operationstechnik mit den üblichen Materialien eines unfallchirurgischen Operationssaales angewandt. Nach marginaler Tumorresektion wurde zur Rekonstruktion der tragenden Gelenkanteile der Defekt mit Knochenzement aufgebaut und mit Metallarmierung versehen bzw. eine zementierte Standard-TEP mit Pfannendachschale implantiert. Während des postoperativen Aufenthaltes, der im Mittel 25 Tage betrug, verstarb niemand. An Komplikationen gab es 2 Wund-infektionen. Alle Patienten waren bei Entlassung voll gehfähig. Eine Reduzierung der Morbidität einhergehend mit einer Besserung der Lebensqualität war in fast alien Fallen möglich. Doxifluridine (5’dFUR) is a new fluropyrimidine derivative with significant antitumor activity in animal models. Preliminary studies with bolus injections, one, six hours and continous infusion, have indicated that doxifluridine is active in a variety of refractory tumors. In a randomized multicentre trial 71 previously untreated patients with advanced colorectal carcinoma were allocated to receive doxifluridine (5’dFUR) 4,000 mg/m2 or 5-fluorouracil (5-FU) 450 mg/m2 as one hour intravenous infusion daily for 5 consecutive days every 4 weeks. In 61 evaluable patients (5’dFUR 31 and 5-FU 30) non-complete remission was treated with 5’dFUR and 2/30 (7%) of patients treated with 5-FU. The median survival of the 5’dFUR arm (all patients) was not statistically different compared to 5-FU (52.5 and 42.5 weeks). Gastrointestinal adverse events were common in both treatment groups. Hematotoxicity occurred in both schedules with a higher incidence for the 5’dFUR arm. Additionally, low grade toxicity (WHO grade 1–2) of skin, neurotoxic symptoms and cardiotoxicity occurred in the 5’dFUR arm only. 5’dFUR in dosage and schedule used in this study is an active agent for advanced colorectal carcinoma but efficacy is accompanied by more pronounced
