&NA;Serious or permanent neurological complications following routine lumbar myelography are uncommon in clinical practice. We describe the sudden and dramatic onset of a symmetrical dense paraparesis in a patient after an uneventful lumbar puncture performed during myelography. &NA;We present an unusual case of a spinal dural arteriovenous malformation (AVM) which produced compression radiculopathy of the left S1 nerve root. The nerve root was compressed by epidural vessels, draining vessels, and the nidus, without the characteristic myelographic findings of a dural AVM. No feeding vessels could be identified. It is important to consider the possibility of a spinal dural AVM in middle‐aged patients who suffer lumbosacral radiculopathy, even if myelography does not demonstrate the characteristic findings. &NA;Two patients with spinal dural arteriovenous malformations associated with intramedullary changes confirmed by the T2‐weighted magnetic resonance imaging (MRI) scans are reported. The characteristics of the MRI findings for these 2 patients were as follows. 1) In the T2‐weighted spin‐echo image, intramedullary changes observed by MRI were visualized as a high signal intensity area at the level where delay in venous circulation of the spinal cord was revealed by the angiography of the spinal cord. 2) After the obliteration of the arteriovenous shunt by surgical management, intramedullary changes remarkably decreased and disappeared with the disappearance of swelling of the spinal cord observed preoperatively. After that, the atrophy of the spinal cord was shown. 3) The level, extent, and severity of intramedullary changes remarkably decided by the condition of the level where the radiculospinal vein, as the flowing vein, refluxes into the coronal venous plexus and venous flow occurs through the radiculospinal vein into the epidural veins. We suspect that intramedullary changes shown by the T2‐weighted MRI scan chiefly represent edema of the spinal cord, caused by an increase in venous pressure due to venous congestion of the spinal cord. MRI is a very useful diagnostic aid to detect intramedullary changes associated with a spinal dural arteriovenous malformation and to evaluate therapeutic results after surgery. &NA;A rare neurovascular complication of a antebrachial arteriovenous fistula in a chronic hemodialysis patient is reported. A large brachial venous pseudoaneurysm caused median neuralgia by direct compression of the nerve. Surgical resection of the pseudoaneurysm resulted in complete relief of neuralgia. &NA;A pseudoaneurysm of the inferior gluteal artery presenting as sciatic nerve compression is reported in a 40‐year‐old woman. Following a transvaginal needle biopsy for endometriosis, the patient developed left sciatic pain and a nonpulsatile mass palpable in the left buttock thought to represent a pyriformis hematoma. Sequential computed tomographic scans were consistent with this diagnosis. Persistent pain and progression of neurological deficits led to surgical exploration. Posterior exposure of the pyriformis muscle and proximal sciatic nerve revealed a large pseudoaneurysm of the inferior gluteal artery compressing the nerve. A laparotomy was performed and the internal iliac artery was ligated, followed by evacuation of the aneurysm contents and repair of the aneurysm neck via a posterior approach. The patient has remained pain‐free with progressive improvement in neurological function after 1 year follow‐up. Aneurysms of the gluteal artery are unusual, predominantly occur after significant pelvic trauma, and rarely present as sciatica. Pertinent aspects of the patient history and clinical findings are atypical for discogenic sciatica. Because of the rarity of this entity, preoperative diagnosis is usually not achieved. Angiography or magnetic resonance imaging should be performed in patients with atypical sciatica and a mass in the region of the proximal sciatic nerve, particularly after trauma. &NA;The histopathology and clinical course of three patients with schwannomas of the brain and high cervical cord after therapeutic irradiation for intracranial malignancy and for ringworm of the scalp are described. Earlier reports in the literature indicated that radiation of the scalp may induce tumors in the head and neck. It is therefore suggested that therapeutic irradiation in these instances was a causative factor in the genesis of these tumors. (Neurosurgery24:929‐932, 1989)

&NA;We describe a rare case of a patient who had a spontaneously ruptured pineal epidermoid cyst concurrent with a thalamic germinoma. Computed tomographic scans led to the detection of both tumors. A definite diagnosis of the free fat in the ventricle was made by its movable feature and characteristic absorption value on computed tomographic scan. Surgical treatment produced a good result. &NA;A case of a 29‐year‐old woman with a cystic lesion in the right cerebellopontine angle, who had three bouts of chemical meningitis in 1 year, is reported. The lesion proved to be a delicate, thin‐walled cyst, in part with a squamous lining and in part with a respiratory epithelial (tall, columnar, ciliated, mucus‐secreting) lining. &NA;The authors report the case history of a patient with a colloid cyst of the 4th ventricle. The clinical history of slow intracranial hypertension, homogeneous hyperdensity with contrast enhancement on computed tomographic, scan, and the gelatinous aspect of the cyst at surgery are characteristic of colloid cysts. The diagnosis was made, however, only at the time of histological examination. A common neuroepithelial origin with 3rd ventricle cysts would be explained by similar embryological developments of the prosencephalic and the rhombencephalic roofs. (Neurosurgery24:939‐942, 1989)
