&NA;Thirteen children and adolescents between ages 4 and 19 years developed transient nonconvulsive neurological symptoms and signs within a few hours of apparently trivial head injuries. Some became quite seriously ill, but all made rapid and complete recoveries. Some of these young people are known to have suffered from migraine before and/or after this episode, and in all but one of the others a family history of migraine was elicited. With these patients are compared five younger children who developed epileptic fits after equally slight trauma. A diagnosis of migraine should be considered in children who develop delayed impairment of consciousness after head trauma, with or without convulsive phenomena or focal neurological deficits, and in whom specialized investigations reveal no sign of a mass lesion or of intracranial hemorrhage.

&NA;Forty‐one intraoperative angiograms were performed on 39 patients undergoing surgical therapy for aneurysms or excision of deep arteriovenous malformations. Good quality radiographs were produced in each case using commonly available equipment. One nonfatal stroke syndrome was the only serious complication possibly attributable to these angiograms. The techniques of opacifying the anterior and posterior intracranial circulations are described and the value of angiography in the intraoperative situation is discussed.

&NA;A case is reported of an arteriovenous malformation in the left motor speech area which was successfully excised without aphasia in a right‐handed man. Preoperative sodium amobarbital carotid infusion testing demonstrated that the patient had right hemisphere dominance for speech. A review of the literature suggests that this is not an uncommon occurrence in individuals with congenital lesions of the left cerebral hemisphere. Lateralization of speech dominance should be eestablished through definitive testing in these cases, especially if surgical removal of the malformation is contemplated.

&NA;A study was made in 10 patients with brain tumors of the effect of methylprednisolone sodium succinate (Solu‐Medrol) on clinical neurological status, intracranial pressure, and periventricular elastance. Significant clinical improvement and reduction in periventricular elastance both occurred within 24 hours of starting treatment, whereas intracranial pressure was not significantly reduced until the 2nd day of therapy.

&NA;The concept of interbody (intercorporal) fusion as a useful treatment for intervertebral disc disease in the cervical area has been well received. Thirty‐two years have passed since Cloward first introduced his technique of posterior lumbar intervertebral fusion. The author believes that the delayed acceptance of this procedure is due to fear of technical difficulties. A technical modification of Cloward's posterior lumbar interbody fusion is introduced. It entails better technique in controlling epidural bleeding by careful positioning of the patient and the use of oxidized cellulose as a tampon in the epidural space. The integrity of the facet is preserved through a more limited interlaminal approach. Osteosynthesis of the grafts is assured by multiple perforations of the cortical plate in accordance with Robinson's principle utilized in cervical interbody fusion. The author believes that the modification simplifies the Cloward posterior lumbar interbody fusion. It also assures better stability after surgery by retention of the facet and lessening the dangers of settlement of the graft by preservation of the cortical plate. In a series of 75 cases, tomograms made 4 months after operation have shown a viable graft with active osteosynthesis between the graft and the adjoining vertebral bodies in 94%.

&NA;Fractures of the 2nd cervical vertebra are frequently undetected on initial evaluation. Tomography may be necessary to delineate the fracture. This is a report of 20 patients treated by internal fixation using interlaminar wiring. There was no operative mortality. Follow‐up studies, an average of 25 months after operation, demonstrated satisfactory results and no evidence of delayed neurological deficit.

&NA;Animal models enabling reliable access to ventricular cerebrospinal fluid (CSF) are crucial to the study of neuropharmacological and neurotoxicological effects of cytotoxic agents used to treat central nervous system neoplasms. This investigation concludes that 4th ventricular catheterization using subcutaneous CSF reservoirs in rhesus monkeys: (a) provides chronic access to sterile CSF without chronic immobilization, (b) enables mixing of injected drugs with lateral ventricular CSF, (c) permits sensitive monitoring of intraventricular pressure and (d) does not produce tissue damage during cannula implantation or breakdown of the blood‐brain barrier.

&NA;A self‐contained tubular drill guard was adapted for use in performing anterior cervical discectomies and interbody fusions. The use of this instrument climinates steps. provides absolute safety against drilling too deeply, allows for adjustment in individual cases, provides for easy inspection of the trephine hole, permits electing the cephalocaudal angle of the drill, and gives a measurement of the ultimate depth of the trephine hole. In 171 consecutive operations only one neurological complication occurred, and total morbidity related to cervical surgery was 6% (7% morbidity was associated with surgery at the donor site). The data suggest that the technical modification herein advocated reduces the surgical complication rate.
